摘要
目的 探讨扩张器的形状和容积对皮肤扩张面积等指标的变化是否有影响。方法 在实验动物家猪埋置不同形状和容积的扩张器进行对比。结果 对不规则的皮损选用一个容积较大的扩张器要比同时使用几个容积较小的扩张器能加速扩张；在同等容积的情况下，椭圆形圆形扩张器比所需时间可减少５０％，囊内压减少２０％；肾形比圆形扩张器所需时间减少１０％，囊内压减少３５％～５０％，而皮肤扩张面积多２．２％。各种形状的扩张器所扩张出皮肤的氧分压和组织学变化差异均无显著意义。结论 使用较大容积的扩张器与使用多个小扩张器，达到同样的扩张面积，前者所需时间短、囊内压低、注水量少，并能减轻创伤、减少感染和注射壶的数量。
Objective To research whether the shape and the size of a tissue expander has an influence on the changes of index of the expanding surface area.Methods Different shapes and sizes of tissue expanders were implanted under porcine skin, and experimentally compared. Result When a defect is irregular, a larger expander is more efficient than several smaller expanders in speeding up the expansion. At the same volume of implant, an oval expander cost 50 % less time and reduced 20 % of intraluminal pressure than a round one; a kidneir shaped expander cost 10 % less time and reduced 35 %～50 % of intracapsular pressure than a round expander while the skin expanded area were increased by 2.2 %. There was no significant difference in the changes of PO 2 and histologic structure of local tissue among all shapes of expander.Conclusion To achive the same expanded area of skin compared with several smaller expanders, a larger tissue expander costs less time, appears a lower intracapsular pressure, needs less volume of water filling, reduces the number of injection pots and has a mild trauma and a lower risk of infection.
