上尿路结石治疗方法五年回顾

Retrospective Investigation for the Treatment of Lithiasis within Upper Urinary Tract in Last 5 Years

目的 提高上尿路结石诊治水平。方法 对 2 43 2例上尿路结石患者的资料作回顾性分析。结果 单个的肾盂肾盏结石和Ⅰ类输尿管结石选择ESWL治疗 ;复杂性肾结石和Ⅱ、Ⅲ类输尿管结石选择开放性手术取石。结论 ESWL治疗后应积极处理石街 ,控制感染和最大限度地减轻医源性肾功能损害。开放性手术要认真提高手术技巧 。

Objective To improve the therapy of lithiasis within upper urinary tract.Method The clinical data for 2432 cases of lithiasis within upper uriary tract between 1994 and 1997 were retrospectively analysed.Results The result indicated that the choice of therapy for single calculi in pyelocalix area and typeⅠ calculus in ureter was ESWL,whereas the choice of complex renal lithiasis and typeⅡ typeⅢ calcule in ureter was lithotomy.Conclusions Special attention should be paid to the treatment of calculus lane,controlling infection and rehabilitation of renal function which might be worsen before and after operation.The level for skill in lithotomy should be raised.To overcome difficulty,bullet curve and lithocenosis might be used,the side effect of operation could be avoid if the internal drainage was settled.
