漳州地区乙型肝炎病毒基因型分布及其临床意义 被引量:4

Characteristics and clinical significance of HBV genotypes distribution in Zhangzhou

摘要 目的了解漳州地区乙型肝炎病毒(HBV)基因型分布情况并探讨其临床意义。方法采用特异性引物以巢式多重聚合酶链反应对271例乙型肝炎患者的血清标本进行基因型检测,同时对部分PCR产物进行测序并确定其基因型。结果 271例慢性乙型肝炎患者中,HBV基因B型228例,占84.1%;C型38例,占14.0%;D型4例,占1.5%;B、C混合型1例,占0.4%。入选患者中未检出A、E、F基因型。B、C基因型在不同程度乙型肝炎患者中的分布无显著差异,与肝炎程度无显著相关性。B、C型肝炎患者的年龄,性别,ALT、AST、TBil、DBil、TP、ALB水平,HBVDNA载量及HBeAg阳性率均无显著性差异。结论漳州地区HBV基因型以B型为主,C型次之,有少量的D型及B、C混合型。B、C基因型与乙型肝炎患者ALT水平、HBVDNA载量、HBeAg阳性率等临床指标无显著相关性。 Objective To study the characteristics and clinical significance of hepatitis B virus(HBV)genotypes distribution in Zhangzhou.Methods All 271 serum samples of patients with chronic HBV infection were selected and detected by nested PCR with multiplex pairs of genotype-specific primers and the positive results were investigated by DNA sequencing in order to detect the genotypes.Results The prevalence of genotype B was 84.1%(228/271)of 271 patients with chronic hepatitis B,type C was 14.0%(38/271),type D was 1.5%(4/271),mixed genotype B and C was 0.4%(1/271),while none of genotype A,E and F were found.There were no significant difference and relation in the distribution characteristics of genotype B and C among various degree of patients with HBV infection.No significant difference was found in the positive rate of HBeAg or the serum level of HBV DNA,ALT,AST,TBil,DBil,TP,ALB or the age,gender among the patients of genotype B and C.Conclusions Genotype B was the major type in patients with HBV infection in this area,genotype C was the medium,genotype D and mixed genotype B and C were both low.There were no significant association between genotypes B,C and ALT level or HBV DNA load,or HBeAg positive rate of patients with chronic hepatitis B.
