退变腰椎间盘中细胞凋亡及DR4、DcR1的表达

摘要 目的检测死亡受体DR4及诱骗受体D cR1的表达、腰椎间盘中髓核组织细胞凋亡率。方法采用原位DNA片段末端标记法(TUNEL)和免疫组织化学方法对退变组的36例退变腰椎间盘髓核组织及对照组的22例创伤腰椎间盘髓核组织进行检测。结果 36例退变腰椎间盘组织细胞凋亡率为52.41%,DR4细胞阳性率为53.24%,D cR1细胞阳性率为24.36%;22例创伤腰椎间盘组织细胞凋亡率为35.47%,DR4细胞阳性率为32.53%,D cR1细胞阳性率为30.54%。两组间细胞凋亡率、DR4蛋白表达差异均有统计学意义(P<0.05)。两组间D cR1蛋白表达差异无统计学意义(P>0.05)。两组髓核内DR4染色阳性细胞率与TUNEL阳性细胞率之间呈正相关(r=0.865,P<0.01)。两组髓核内D cR1阳性细胞率与TUNEL阳性细胞率无关,差异无统计学意义(P>0.05)。结论腰椎间盘中可能存在TRAIL/DR4诱导的凋亡途径,DR4可能在腰椎间盘退行性变发生和发展中发挥重要作用。 Objective To determine the apoptosis rate and the expressions of DR4 and DcR1 in human lumbar intervertebral discs.Methods We detected the apoptosis rate and the expression of DR4 and DcR1 in 36 cases of degenerative lumbar intervertebral disc tissues and 22 of traumatic normal lumbar intervertebral disc tissues with TdT-mediated dUTP biotinnick end labeling(TUNEL) and immunohistochemical staining methods.Results The average apoptotic index(AI) and The expressions of DR4 protein in degenerative lumbar intervertebral discs was significantly higher than traumatic normal cases(52.41%vs 35.47%,P0.05;53.24%vs 32.53% P0.05).There was no difference in the expression of DcR1 protein between the two groups(24.36%vs 30.54%,P0.05).The DR4 positive incidence of nucleus pulposus showed significant positive correlation with the TUNEL positive incidence of nucleus pulposus(r=0.865,P0.01).The DcR1 positive incidence of nucleus pulposus was not related with TUNEL positive incidence of nucleus pulposus(P0.05).Conclusion DR4 may play an important role in the occurrence and progression of lumbar intervertebral disc degeneration,The apoptosis pathway induced by DR4/tumor necrotic factor-related apoptosis-inducing ligand(TRAIL)maybe exists in LID tissues.
