应变率成像技术评价左室非正常充盈模式患者左房功能的研究 被引量:3

摘要 目的探讨应变率成像(SRI)技术评价左室非正常充盈模式患者左房功能的临床应用价值。方法选取我院57例左室非正常充盈模式患者和21例除外心血管系统疾病的正常对照组,根据左室充盈模式将其分为正常充盈组、松弛性下降组、假性充盈正常组和限制性充盈异常组,使用SRI技术检测并比较分析各组左房各壁收缩期、舒张早期、舒张晚期应变率峰值(SSR、ESR、ASR)。结果各组间ESR比较差异均有统计学意义(P<0.05),且假性充盈正常组小于正常充盈组而大于其余组;除假性充盈正常组与正常充盈组外,各组间ASR比较差异均有统计学意义(P<0.05),且松弛性下降组值最大;除松弛性下降组与假性充盈正常组外,各组间SSR比较差异均有统计学意义(P<0.05),且随左室舒缩功能受损发展而递减。结论 SRI可定量评估左房功能,间接定量反映左室舒缩功能改变。 Objective To explore the clinical application value of strain rate imaging (SRI) in evaluating left atrial function in patients with abnormal filling patterns of left ventricle. Methods Fifty - seven patients with abnormal filling patterns of left ventricle and 21 normal subjects were enrolled in this study. They were divided into normal filling group, relaxation drop group, pseudonormal filling group and restrictive filling group. Then the strain peak systolic strain rate ( SSR), early diastolic strain rate peak (ESR) and late diastolic strain rate peak (ASR) of atrial septum were measured by SRI. Results There was significant difference of ESR between each group ( P 〈 0.05 ), and ESR in pseudonormal filling group was lower than that in normal fillling group. Except for comparison between normal group and pseudonormal filling group, there was significant difference of ASR between each group ( P 〈 0.05 ) , the value of late diastolic strain rate peak in relaxation drop group was increased while droped in other abnormal filling groups. Except comparing between relaxation drop group and pseudonormal filling group, there was significant difference of SSR between each group ( P 〈 0.05 ) , and it was decreased gradually with left ventricular function damage. Conclusion SRI can quantitatively evaluate left diastolic function in patients with abnormal filling patterns of left ventricle and indirectly quantify left ventricular function change.
