摘要
目的探讨2型糖尿病(T2DM)患者血清8-羟基脱氧鸟苷酸(8-OHdG)与胰岛素抵抗(IR)和颈动脉内膜中层厚度(IMT)的相关性。方法 50例初诊T2DM患者根据IMT分成A组(IMT<1.0 mm,32例)和B组(IMT≥1.0 mm或存在硬化斑块,18例);正常对照组(C组,25例)。检测三组血清8-OHdG水平、体重指数(BMI)、空腹血糖(FBG)、血脂、胰岛素(Ins)、糖化血红蛋白(HbA1c),计算胰岛素抵抗指数(Homa-IR)。结果 A组和B组的血清8-OHdG水平均显著高于C组,且B组较A组显著升高(P<0.05);血清8-OHdG水平与Homa-IR和IMT呈正相关(P<0.05)。8-OHdG、Homa-IR、HbA1c、总胆固醇(TC)为IMT的风险预测因子。结论血清8-OHdG水平增高与IR和颈动脉IMT的增厚相关,提示氧化应激参与了IR的形成及T2DM并发症的发生。
Objective To investigate the correlation between serum 8-hydroxy-guanine(8-OHdG) and insulin resistance (IR) and intima-media thickness(IMT) of the carotid artery in patients with type 2 diabetes mellitus(T2DM).Methods The concentrations of fasting blood glucose(FBG),blood lipid,insulin(Ins),glycolated hemoglobin(HbA1c) and serum 8-OHdG were measured and body mas index and insulin resistance index(Homa-IR) were calculated in 25 healthy subjects(group C) and 50 T2DM patients,who were divided into groups of A(32 cases,IMT1.0 mm) and B(18 cases,IMT≥1.0 mm or having atheromatous plaque).Results Serum 8-OHdG was significantly higher in groups of A and B than that in group C,which was significantly higher in group B than that in group C(P0.05).Serum 8-OHdG level was positively correlated with Homa-IR and IMT.Stepwise multiple regression analysis showed that 8-OHdG level,Homa-IR,HbA1c and TC were the risk factors for IMT.Conclusion The increased level of serum 8-OHdG is correlated with insulin resistance and increased IMT of the carotid artery,indicating that oxidative stress participates in the formation of insulin resistance and occurrence of diabetic complications.
