摘要
目的:观察动力髋与动力髋加防旋螺钉在固定股骨转子间骨折时生物力学强度的差别。方法:取成年男性防腐股骨标本12对,将每对标本制造成不稳定的Evans-JensenⅢ型股骨转子间骨折,左侧用4孔动力髋螺钉而右侧4孔动力髋加防旋螺钉固定。进行压力和扭力试验,测量骨折间位移,比较是否有差别。结果:当压力加载至1500牛顿时,动力髋与动力髋加防旋螺钉固定的股骨转子间骨折处移位比较差异无统计学意义(P>0.05);在扭力实验中,当至18牛.米扭转量程,动力髋与动力髋加防旋螺钉固定的股骨转子间骨折处移位比较差异有统计学意义(P<0.05)。结论:动力髋与动力髋加防旋螺钉在固定EvansⅢ型股骨转子间骨折时,压力强度下无差别,扭力强度下有差别。
Objective:To determine the biomechanical strength of a dynamic hip screw with preventing rotary screw for the reconstruction of unstable intertrochanteric fractures.Methods:12 pair's femoral specimens from adult male were used to make the Evans-Jensen Ⅲ unstable intertrochanteric femoral fracture model.All fractures were fixated with the DHS and a four-hole side plate in the left;a DHS with a four-hole side pate with proventing rotary screw were used in the right.Then the pressure and torque were performed in the entire fracture model,the distance of the fracture was measured,the data was analysised.Result:There is not significant difference in the distance of the fracture between the four-hole side plate and four-hole side-plate with proventing rotary screw(P0.05),when the pressure is 1500 Neton,There is significant difference in the distance of the fracture between the four-hole side plate and four-hole side-plate with proventing rotary screw,when the torque pressure is 18 N·M(P0.05,in the rotation test.Conclusion: There is no significant difference between the four-hole side plate and four-hole side-plate with proventing rotary screw in the pressure test,but there is significant difference in the rotation test.
