摘要
目的比较三种蒽环类抗癌药治疗非何杰金淋巴瘤的近期疗效和毒性反应。方法用BACOP和CHOP方案治疗非何杰金淋巴瘤共145例。BACOP方案用法为ADM25mg/m2或THP25mg/m2或EPI30mg/m2,iv,d1、8;CTX650mg/m2,iv,d1、8;VCR1.4mg/m2.iv,d1、8;PYM8mg,im,d1、3、8、10;PDN40mg/m2,po,d1—14,28d为1周期。CHOP方案为ADM40mg/m2或THP45mg/m2或EPI50mg/m2,iv,d1;CTX750mg/m2,iv.d1;VCR1.4mg/m2,iv.d1;PDN60mg/m2.po,d1—5,21d为1周期。结果ADM.THP.EPI组CR率分别为37.1%、41.3%、48.6%.CR+PR率分别为71.0%、80.4%、83.8%,P>0.05,ADM组心脏毒性和胃肠道反应明显高于THP组和EPI组,P<0.05,THP组和EPI组近期疗效和毒性反应相似,但THP价格和用量均低于EPI。结论THP可替代EPI治疗非何杰金淋巴瘤.尤其适用于老年患者。
Objective To compare the short-term efficacy and toxicity of three kinds of anthracylcines for non-Hodgkin's lymphoma. Methods 145 patients with non-Hodgkin's lymphoma were treated with BACOP regimen or CHOP regimen. BACOP regimen consisted of adriamycin (ADM ) 25mg/m2 or pirarubicin (THP) 25mg/m2 or epirubicin (EPI ) 30mg/m2 IV,on day l、8; cyclophosphamide (CTX ) 650mg/m2 IV, on day 1、8; vincristine (VCR ) 1. 4mg/m2 IV, on day 1、8; bleomycinA5 (PYM ) 8ing IM. on day 1、3、8、10; prednisome (PDN ) 40mg/m2 PO, on day 1-14, 28 days formed a course of treatment. CHOP regimen consisted of ADM 40mg/m2 or THP 45mg/m2 or EPI 50mg/m2 IV,on day 1; CTX 750mg/m2 IV,on day 1;VCR 1. 4mg/m2 IV,on day 1; PDN 60mg/m2 PO.on day 1-5, 21 days formed a course of treatment. Results The complete response rates in ADM group, THP group and EPI group were 37. 1 %、41. 3%、48. 6% respectively,and the overall response rates were 71. 0%、80. 4 %、83. 8 % respectively, (P>0. 05). Cardiac toxicities and the gastrointestinal tract reactions were mostly seen in ADM group among three groups. (P<0. 05). The short-term efficacy and toxicity of THP group was similar to that of EPI group,but the price and dosage of THP were lower than those of EPI. Conclusion THP can substitute for EPI in treating NHL and is especially suitable for the elderly patients.[
