三七总皂甙注射液治疗肺源性心脏病急性发作期心力衰竭患者的疗效观察

Observation on curative effects of panax notoginseag total saponin injection on patients with heart failure induced by acute seizure phase of cor pulmonale

目的:探讨三七总皂甙注射液对肺源性心脏病(肺心病)急性发作期心力衰竭(心衰)患者的疗效。方法:60 例肺心病急性发作期心衰患者随机分为2 组,对照组30 例采用吸氧、抗生素、皮质激素等西医治疗;治疗组30 例在此基础上,加用三七总皂甙注射液静滴,每日1 次,2 组均连续治疗15 日。结果:治疗组显效率56.7% ,总有效率93.3% ,显著高于对照组(分别为36.7% 和76.7% ,χ21= 9.9,P< 0.01 和χ22= 4.3,P< 0.05)。2 组治疗后平均肺动脉压(PAMP)、心排血量(CO)、肺血管阻力(PVR)、总外周阻力(TPR)、动脉氧分压( PaO2)、二氧化碳分压(PaCO2 )和血氧饱和度(SaO2)均较治疗前明显好转(P 均< 0.01)。治疗后治疗组PAMP、PVR、TPR、PaO2、SaO2 均较对照组显著增高(P 均< 0.01)。结论:三七总皂甙注射液具有扩张血管、改善微循环、降低血管阻力的作用,治疗肺心病心衰有较好疗效。

Objective:To investigate the influences of panax notoginseag total saponin injection (PNTS) on patients with heart failure induced by acute seizure phase of cor pulmonale.Methods:60 patients were randomly divided into two groups.The control group ( n =30) was treated with western medicine including inhalation of oxygen,antibiotic,corticosteroid and so on while in treated group ( n =30) based on the same therapy the PNTS injection infused intravenously once a day was added for 15 days continuously to both two groups. Results: The markedly effective rate (56 7%)and the total effective rate (93 3%) in treated group was significantly superior to those (36 7% and 76 7%) in control group (χ 2 1=9 9, P <0 01;χ 2 2=4 3, P <0 05).In two groups after therapy the improvements of all of the mean palmonary artery pressure (PAMP),the cardiac output (CO),the pulmonary vascular resistance (PVR),the total peripheral resistance(TPR),the PaO 2,the PaCO 2,and the arterial oxygen saturation(SaO 2) were much more significant than those before therapy (all P <0 01 ).After therapy in treated group the increases in PAMP,PVR,TPR,PaO 2,and SaO 2 were much more obvious than those in control group (all P <0 01).Conclusions:PNTS injection possesses the actions pH dilating vessels,improving microcirculation,and depressing vascular resistance and better curative effects in treating cor pulmonale complicating cardiac failure.
