原发性肺癌危险因素及其人群归因危险度的研究 被引量:4

摘要 目的 :探讨与原发性肺癌发生有关的因素 ,为预防肺癌提供科学依据。方法 :采用 1∶ 2配对病例 -对照研究方法进行研究 ,应用多因素条件 L ogistic回归模型分析与肺癌发生有关的因素 ,并对肺癌危险因素的人群归因危险度 ( PAR)进行估计。结果 :筛选出 8个与肺癌有关的因素 ,其中吸烟、离退休、工作场所有毒物质、肺结核病史、常做饭菜、使用煤炭等因素增加肺癌的危险性 ,其危险度 ( OR)分别是 :2 .3 78,2 .3 18,2 .2 62 ,1.975 ,1.793 ,1.75 6,人群归因危险度分别是 :3 8.0 9% ,2 3 .3 0 % ,19.2 0 % ,13 .76% ,2 3 .2 1% ,15 .5 3 % ,6项危险因素总的人群归因危险度为 76.95 % ;而居室通风良好、常食用水果能降低肺癌的危险性 ,其危险度 ( OR)分别是 :0 .5 17,0 .70 2。结论 :吸烟、离退休、工作场所存在有毒物质、有肺结核病史、常做饭菜、使用煤炭等是肺癌的危险因素 ,如果消除这些因素可使人群中肺癌的发生减少 76.95 % ;而居室通风良好。 Objective:In order to investigate the factors associated with primary lung cancer, and calculate the population attributable risk for risk factors of lung cancer Methods:A 1∶2 matched case control study was carried out, which involved 122 cases and 244 controls, and the data was analysed by using condition Logistic regression model,meanwhile,population attributable risk (PAR) was estimated by means of Logistic regression method Results:According to the multiple condition Logistic regression model,smoking,retirement,professional contact with toxic substance, history of pulmonary tuberculosis,cooking,indoor burning coal increased the risk of lung cancer,the OR value for these items is 2 378,2 318,2 262,1 975,1 793,1 756,respectively,and PAR for each of them is 38 09%,23 30%,19 20%,13 76%,23 21%,15 53% respectively,the total PAR is 76 95 % Consistent intake of fresh fruits,house airiness had a protective effect,the OR value is 0 507 and 0 701 Conclusion:Smoking,retirement,professional contact with toxic substance,history of pulmonary tuberculosis,cooking,indoor burning coal are risk factors of lung cancer;house airiness and consistent intake of fresh fruits,are protective factors of lung cancer
