N D P K/nm 23 及 C D K N2/ P16 蛋白在卵巢癌中的表达

摘要 目的:探讨抑癌基因 P16 及转移抑制基因nm 23 与卵巢上皮性肿瘤的发生、发展及淋巴结转移的关系。方法:免疫组化 S P法检测12 例正常卵巢,14 例卵巢上皮性良性肿瘤及52 例卵巢癌组织中 P16 蛋白和nm 23 蛋白的表达。结果:nm 23 蛋白在卵巢癌组织中阳性率为67.3% ,明显高于正常卵巢及良性肿瘤组织中的表达率(41.7% ,42.8% ), P< 0.05。在浆液性癌(82.1% )和粘液性癌(50.0% )中的阳性率也有显著差异, P< 0.05,特别是在有淋巴结转移(39.1% )和无淋巴结转移(89.7% )的卵巢癌中检出率相差非常显著, P< 0.01。nm 23 蛋白阳性率与 O E C的临床分期及病理分级相关不显著。 P16 蛋白在卵巢癌中检出率为28.8% ,明显低于正常卵巢(75% )和良性上皮性肿瘤(57.1% ), P< 0.01,在晚期肿瘤中的表达率(22.2% )也远低于早期肿瘤(71.4% ),而且在高度恶性组织中表达水平(17.0% )与中低度恶性组织的表达水平(44% )之间也有显著差异, P< 0.05。二者均与患者年龄无关。结论:nm 23、 P16 基因与 O E C的发生、发展密切相关,nm Objective: To explore the relationship between the expressions of tumor metastasis suppressor gene NDPK/nm23 or tumor suppressor gene CDKN/P16 and the occurrence, development and lymph node metastasis of ovarian epithelial carcinoma. Methods: The expression of P16 protein and nm23 protein were detected by streptavidin peroxidase conjugated method in 78 cases of tissue samples including 12 normal ovary,14 ovarian benign epithelioma and 52 OEC. Results: The positive rate of nm23 H1 expression was 67.3%in the tissues of OEC which was higher than that of the normal ovary(41.7%) and benign ovarian epithelioma (42.8%)( P <0.05).Significant difference between the positive rates in serous carcinomas (82.1%) and mucinous carcinomas (50.0%)( P <0.05) was observed. The positive expression rate of nm23 H1 in OEC was especially related to the lymph node metastasis rather than the clinical stage or the pathologyical grades. The positive expression rate of P16 in OEC was 28.8% significantly lower than that of normal ovary(75.0%) and benign ovarian epithelioma (57.1%)( P <0.01).The positive expression rate of P16 in the advanced cases was 22.2%, markedly lower than that of the early cases(71.4%) and of the the rate in poorly differentiated cases was 17.0% significanqtly lower than that high grade cases(44%).No relationship was found between the expression of nm23 and P16 and the ages of the cases. Conclusion: Nm23 and P16 are closely associated with the occurrence and development of OEC and nm23 play a very important role in OEC lymph node metastasis.P16 protein may be an important marker for the prognosis of OEC.
