64层螺旋CT三维减影技术在颈脑血管成像中的可行性研究 被引量:7

Feasibility of 3D Subtraction Technique of 64-slice CT in Cervical and Cerebral Angiography

摘要 目的应用64层螺旋CT数字减影技术进行颈脑血管成像,以探讨东软医疗公司研发的三维减影软件的可行性。资料与方法对71例患者行颈脑血管数字减影CTA检查,得到平扫和常规CTA图像;其中49例同时接受数字减影血管造影(DSA)检查。先利用GE工作站Add/Sub软件进行二维减影得到二维减影CTA图像;然后采用东软医疗公司研发的BSCTA软件进行三维减影得到三维减影CTA图像。由一名高年资放射科医师记录并比较常规CTA、二维减影CTA、三维减影CTA进行容积再现(VR)及最大密度投影(M IP)后处理的时间,同时由两名高年资放射科医师采用双盲法比较三种CTA技术重组所得颈脑血管图像质量,并与DSA比较。结果经单因素方差分析两两比较,三种CTA技术后处理时间两两之间差异均有显著统计学意义(P<0.05),三维减影CTA最短,二维减影CTA次之。三种CTA技术进行三维重组所得颈脑血管图像评分经过非参数Mann-Whitney U检验,两两之间差异均有显著统计学意义(P<0.05),三维减影CTA图像质量最佳,二维减影CTA次之。49例同时接受DSA检查,结果与三维减影CTA一致。结论 64层螺旋CT颈脑血管成像数字减影技术省时、图像质量好,尤其是东软医疗公司研发的三维减影软件的临床应用是切实可行的,其作为一种新的减影软件有着广阔的应用前景。 Objective To explore the feasibility of the 3D subtraction software exploited by the medical company of neusoft by means of using the digital subtraction technique of 64-silce CT in cervical and cerebral angiography.Materials and Methods 71 patients were performed with digital subtraction CT angiography(CTA) in cervical and cerebral vascularities,which brought about the plain CT and conventional CTA images.49 patients of which were undertaken by DSA.First,the Add/Sub software of GE workstation was used for 2D subtraction,and then the BSCTA software exploited by the medical company of neusoft was used for 3D subtraction.One experienced radiologist recorded and compared the post-processing time of VR and MIP among conventional CTA,2D subtraction CTA and 3D subtraction CTA.Meanwhile,the image quality of cervical and cerebral arteries reconstructed from the raw CT data set of the three kinds of CTA techniques were separately compared by two experienced radiologists in a blind way,and then were compared with those of DSA.Results By ANOVA,there were statistically significant difference of the post-processing time between two of the three kinds of CTA techniques(P0.05),the 3D subtraction CTA was the shortest of the three,followed by 2D subtraction CTA.By Mann-Whitney U-test,the grading score on the manifestation of cervical and cerebral arteries between two of the three kinds of CTA techniques had statistically significant difference(P0.05),the image quality of 3D subtraction CTA was the best of the three,which had the same result obtained in DSA;followed by 2D subtraction CTA.Conclusion Digital subtraction technique of 64-slice CT in cervical and cerebral angiography has the merits of time-saving and good image quality,especially the clinical application of 3D subtraction software is feasible.It has a wide application prospect as a new subtraction software.
