摘要
目的:研究河豚毒素不敏感型(tetrodotoxin-resistant,TTX-R)hNav1.8钠通道蛋白(hNav1.8)在三叉神经痛(trigem inalneuralgia,TN)患者痛支神经中的表达水平,探讨TN的发病机制并为TN的药物治疗提供新的靶标。方法:人工合成具有Nav1.8抗原性的短肽,免疫新西兰大白兔得到抗体,以12例原发性TN第三支痛保守治疗无效行手术治疗患者的下牙槽神经及舌神经为研究对象,以舌颌颈联合根治术患者的正常下牙槽神经为阴性对照,肌肉组织1例为正常对照,鼠背根神经节为阳性对照,与制备的hNav1.8钠通道蛋白抗体进行免疫组化反应,对免疫组化结果进行图像分析。结果:hNav1.8钠通道蛋白在TN患者痛支神经中存在明显的表达,而正常下牙槽神经及肌肉组织中无表达。结论:hNav1.8钠通道蛋白在TN患者痛支神经中的异常表达可能与TN的发病机制有关。
Objective: To study the pathogenesis of trigeminal neuralgia (TN) by examine the expression of tetrodotoxin-resistant hNal. 8 sodium channel protein( hNav 1.8) in affected nerves of patients with TN. Methods: 2 New Zealand white rabbits were immunized by short peptides from C terminal of rat Navl. 8 to obtain polyelone antiserum. Twelve samples of affected inferior alveolar nerve and lingual nerve were obtained from patients with idiopathic NT, to whom the drug therapy was not effective, were studied. As negative control, one normal inferior alveolar nerve was obtained from a patient underwent radical neck dissection with glosseetomy and mandibulectomy. One muscle sample was obtained as normal tissue control. A dorsal root ganglion from a rat was obtained as positive control. These tissues and prepared hNal. 8 antibody were used in immunohistoehemistry assay. The positive particle areas of the im munohistoehemistry response were determined by KS400 image analysis system. Results: The expression of hNav 1.8 channel protein was found in all of the twelve specimens of the affected nerves of patients with TN, but not in the muscle sample and the normal inferior alveolar nerve. Conclusion: The abnormal expression of hNavl. 8 channel protein in the affected nerves of patients with TN may play an important role in the pathogenesis of TN.
