重组人表皮生长因子治疗儿童手足口病口腔溃疡的临床疗效观察

Effect of recombinant human epidermal growth factor in the treatment of oral ulcer in children with hand-foot-mouth disease

目的:探讨外用重组人表皮生长因子(金因肽喷雾剂)治疗儿童手足口病口腔溃疡的疗效。方法:将200例患儿随机分为对照组、碘甘油组、冰硼散组和金因肽组,对照组给予口腔常规3%H2O2和生理盐水交替冲洗清洁创面,溃疡面涂抹盐酸丁卡因凝胶;碘甘油组常规清洁创面后,溃疡面涂抹碘甘油;冰硼散组常规清洁创面后,溃疡面涂抹冰硼散;金因肽组常规清洁创面后,溃疡面喷金因肽药液。观察4组患儿即时感觉、疼痛改善时间、溃疡愈合时间和药物的不良反应。结果:治疗总有效率碘甘油组(38%)、冰硼散组(54%)、金因肽组(94%),与对照组(12%)相比差异有显著性(P<0.01),其中金因肽组的总有效率明显高于碘甘油组、冰硼散组(P<0.01);在口腔局部症状改善时间上碘甘油组、冰硼散组和金因肽组明显高于对照组(P<0.01);其中金因肽组的疗效与碘甘油组和冰硼散组相比差异有显著性(P<0.01)。3组用药后均未发现有不良反应。结论:外用重组人表皮生长因子对促进儿童手足口病口腔溃疡愈合效果显著,使用方便安全,值得在临床中推广应用。

Objective： To investigate the effectiveness of recombinant human epidermal growth factor（geneitim） in the treatment of oral ulcer in children with Hand-foot-mouth disease. Methods： 200 children patients with Hand-mouth disease were classfied into 4 groups with 50 in each group. In control group, regular 3% H2O2 and normal saline were used to cross-rinse the ulcer, dicaine geal was daubed on the ulcer. In iodine glycerin group, iodine glycerin was daubed on the ulcer after z^gular cleaning. In Bin-Peng-San group, Bing-Peng-San was daubed on the ulcer ＇after regular cleasing. In genetim group, genetim was sprayed on the ulcer after regular cleansing. The instant feeling, pain ease time, ulcer healing time and drug side effects were observed and analysed. Results： The overall treatment effectiveness rate was 48% in iodine glycerin group, 54% in Bing-Peng-San group, 94% in genetim group and 12% in control group（ P 〈 0.01 ）. The overall treatment effectiveness rate in genetim group was significantly higher than that in iodine glycerin group and Bing-Peng-San group（ P 〈 0.01 ）. Oral partically symptom improvement in the 3 treatment groups was significantly better than timt ill control group（P 〈 0.01 ）. No side-effect was observed after medicine treatment in the groups. Conclusions： Recombi- nant human epidermal growth factor is effective in the treatment of oral ulcer in children with Hand-foot-mouth disease.
