摘要
目的 探讨生长抑素对急性胰腺炎(AP) 血液流变学的影响及其与胰、胰外器官损害程度的相关性。 方法 SD大鼠30 只随机分为假手术组(A组,10 只) 、AP组(B组,10 只)、AP加生长抑素治疗组(C组,10 只) ,观察各组血液流变学、酶学、胰及肾、肺、肠形态学改变及其相关性。 结果 AP组,低切下全血粘度9-4±0-8、红细胞聚集指数2-37±0-11、红细胞刚性指数4-37 ±0-19,胰及肾、肺、肠评分或分级病损严重;治疗组上述各血液流变学指标依次为7-7 ±0-5、2-19 ±0-14、4-02 ±0-12(与AP组比P<0-01) ,胰及胰外器官病损减轻(P< 0-01);统计双变量等级相关分析显示:AP时,血液流变异常程度与胰及胰外多器官损害程度存在显著正相关(P<0-05)。 结论 AP时,血液流变异常是导致胰、胰外器官损害的重要病理生理基础;生长抑素能有效地改善AP血液流变异常。
Objective To study the effect of somatostatin on blood rheology in murine acute pancreatitis (AP). The relationship between the abnormality in blood rheology and the pathologic damages of the pancreas as well as extra pancreatic organs was also studied.Method SD rats were randomly divided into three groups:the sham operative group,the AP group and AP with somatostatin therapy group. The blood rheology changes,the enzymatic levels and the pathology alterations of pancreas,kidney, lung and intestine were measured and recorded in each group.Result In AP group, the whole blood viscosity at low shear rate was 9 41±0 80,the agglutination index of red blood cell was 2 37±0 11, the rigidity index was 4 37±0 19, the scoring or grading of pancreas and kidney,lung,intestine showed the damages were severe.In Ap with somatostatin therapy group, the blood rheology indexes were 7 67±0 52,2 19±0 14, and 4 02±0 12 ( P <0 01),the pathologic damages of pancreas and extra pancreatic organs were all less severe ( P <0 01), the rank correlation analysis showed a positive relationship between the blood rheology abnormality and the multiple organ damage during AP ( P <0 05). Conclusion During the course of AP, blood rheology abnormality was the basis of pathophysiology leading to the lesions seen in the pancreas and extra pancreatic organs.Somatostatin ameliorated the blood rheology abnormality and showed a protective effect against multiple organ damage during AP.
