摘要
目的:为了在心肌灌注显像的同时获得心功能参数,提高冠心病诊断的准确率,我们采用首次通过法心肌显像对51例冠心病患者和健康对照组进行了检查。方法:在负荷高峰时“弹丸”式静脉注射示踪剂,即刻以帧/50m s速度连续采集。设立左室感兴趣区,以左室计数最高一帧为舒张末期,计数最低一帧为收缩末期应用计算机相应程序对其间16 帧~20 帧进行处理得出各项心功能参数。结果:潘生丁负荷下对照组LVEF值与冠心病组LVEF值比较差异显著:潘生丁负荷心肌显像中左室各心肌节段的放射性摄取率与首次通过心功参数中相应心肌节段的REF值和RWM 值有良好的相关性;潘生丁负荷前后心肌显像与首次通过心功参数二者比较,诊断冠心病的灵敏度无明显差异,二者结合后诊断灵敏度可提高至89.4% ,准确率提高至93.3% 。结论:潘生丁负荷前后心肌显像结合首次通过心功参数可使冠心病诊断准确率明显提高。首次通过心肌显像对于判断反向分布是否具有临床意义方面较单纯心肌显像具有较大优势。
Purpose:With dipyridamole stress and first pass myocardial perfusion imaging,we can also get the message of perfusion and function andraise the sensitivity and specificity of diagnosing for CAD.Methods:At the peak of dipyridamole stress,we injected 99mTc-MIBI into the vertery of person with instant,and acquired image immediately.Having finished acquire,we can get series cardiac functional date from computer.Result:The result shows that a significant correlation has shows between the activity absorb radio of abnormal myocardial sections and regional eject fraction and regional wall motion of the same myocardial sections.It is no significant of sensitivity between dipyridamole myocardial imaging(before and after stress)and first pass cardiac function.Conclude:We conclude that dipyridamole stress in combination with first pass myocardial perfusion imaging for assessment of perfusion and function serves as a valuable tool to identify patients with coronary arterial diseases.For the reverse distribution on myocardial perfusion imaging,it has clinical value or not,the regional cardiac function may be an important point for diagnosis.
