新生儿窒息血清抗脑抗体和心肌酶的变化及意义 被引量:1

THE CHANGES AND SIGNIFICANCE OF SERUM ABAb AND MYOCARDIAL ENZYMES IN NEW BORNS WITH ASPHYXIA NEONATORUM

摘要 目的探讨新生儿窒息后血清抗脑抗体和心肌酶水平动态变化对诊断新生儿窒息和判断窒息后心脑损伤的价值。方法将108例窒息新生儿按窒息程度分为重度窒息组(48例)和轻度窒息组(60例),另外选取30例正常足月新生儿作正常对照组,分别于生后12、24、72h取外周静脉血血清-70℃低温保存,用酶联免疫吸附法及速率法分别检测血清抗脑抗体和心肌酶的浓度。结果生后12、24、72h窒息新生儿组血清心肌酶水平较对照组明显升高(P<0.01),且重度窒息组上述指标明显高于轻度窒息(P<0.01);生后24、72 h窒息新生儿组血清抗脑抗体水平较对照组明显升高(P<0.01),且重度窒息组上述指标明显高于轻度窒息(P<0.01)。结论血清抗脑抗体和心肌酶水平可作为新生儿窒息后脑损害和心肌损害的早期监测和诊断的指标,为早期治疗提供依据。 Objective To investigate the changes of serum antibrain- antibody (ABAb) and myocardial enzymes myocardial and brain in new borns with asphyxia and evaluate their significance in the identification of damage. Methods A total of 108 newborns with asphyxia were divided into severe asphyxia(48 cases) and mild asphyxia (60 cases), and 30 normal newborns were chosen as a control group. Peripheral vein blood was collected at 12,24,72h and 7d after birth. The serum was conserved - 70℃ . Serum ABAb and myocardial enzyme were measured by enzyme -linked immunoabsorbent assay (ELISA) and kinetic rate assay. Results At 12,24 and 72 h after birth, the results of myocardial enzymes showed that there were significant differences between control group and asphyxiated group ( P 〈 0. 01 ) , and there were significant differences between mild asphyxiated group and severe asphyxiated group too ( P 〈 0. 01 ) ; At 24 h and 72 h after birth, the results of serum ABAb showed that there were significant differences between control group and asphyxiated group ( P 〈 0. 01 ), and there were significant differences between mild asphyxiated group and severe asphyxiated group too ( P 〈 0. 01). Conclusion Neonatal asphyxia would lead to myocardial and brain damage, the increasing of myocardial enzyme and ABAb levels can be used as an early diagnosis method of myocardial and brain damage and provide basis for early treatment.
