亚急性重型肝炎患者TNF-α和IL-1表达及IL-4对其的调控

Study on effect of IL 4 on expression of TNF α and IL 1 from patients with subfulminant hepatitis

目的：分析ＩＬ４对亚急性重型肝炎（亚重肝）患者外周血单个核细胞（ＰＢＭＣｓ）ＴＮＦα和ＩＬ１的调控作用，评价ＩＬ４在亚重肝治疗中的潜在价值。方法：应用细胞生物学、免疫组化和ＲＴＰＣＲ等方法测定ＰＢＭＣｓＴＮＦα、ＩＬ１的表达。结果：亚重肝患者ＰＢＭＣｓＴＮＦα、ＩＬ１表达水平均较正常人显著增高，虽然ＩＬ４ｍＲＮＡ及蛋白质的表达与正常人比较无显著性差异，但ＩＬ４ｍＲＮＡ／ＴＮＦαｍＲＮＡ和ＩＬ４ｍＲＮＡ／ＩＬ１αｍＲＮＡ的比值明显降低，ＩＬ４以剂量依赖方式明显抑制ＬＰＳ诱导的亚重肝患者ＰＢＭＣｓＴＮＦα、ＩＬ１的表达；与正常人比较，ＩＬ４对亚重肝患者ＰＢＭＣｓ的作用效应降低。结论：ＩＬ４对亚重肝ＰＢＭＣｓＴＮＦα、ＩＬ１的表达均具有明显抑制作用。

Objective: To study the effect of IL 4 on expression of TNF α and IL 1 by peripheral blood mononuclear cells (PBMCs) from patients with subfulminant hepatitis (SFH) and estimate the potential clinical value of IL 4 in treatment of SFH. Methods:The expression of TNF α and IL 1 by PBMCs was detected by biological activity assay,immunohistochemical assay and semiquantitative reverse transcriptional polymerase chain reaction (RT PCR).Results:The expression of TNF α and IL 1 by PBMCs from patients with SFH were greatly enhanced as compared with that of normal controls. Although there was no difference in expression of IL 4 mRNA/IL 1α mRNA and IL 4 mRNA/TNF α mRNA in SFH were lower than in normal controls.IL 4 in dose dependent manner down regulated the LPS stimulated produced of both TNF α and IL 1 by PBMCs from patients with SFH. It was also found that the response of PBMCs to IL 4 decreases in SFH as compared with the normal controls.Conclusion:IL 4 seems to be a potent down regulator of TNF α and IL 1 and may have a potential role in treatment of SFH.
