ICU呼吸机相关性肺炎病原菌与耐药性分析 被引量:5

摘要 目的分析我院综合性ICU内呼吸机相关性肺炎(VAP)病原菌分布构成及其耐药情况,为临床经验性用药提供参考。方法回顾性研究我院ICU 2009年1月~2010年12月收治的需机械通气患者250例,对确诊VAP的96例患者分离到的病原菌进行鉴定、药敏试验。结果共培养出病原菌152株,G-杆菌67.76%(103/152)为主,其中前三位分别为鲍曼不动杆菌、铜绿假单胞菌、大肠埃希菌。G+球菌14.47%(22/152),MRSA是主要革兰阳性菌。真菌13.16%(20/152);药敏试验显示阴性菌对亚胺培南、抗β-内酰胺酶活性的含酶抑制剂的复合制剂有较好的敏感性,阳性菌对万古霉素全敏感。结论 G-杆菌仍是VAP最主要的病原菌,以鲍曼不动杆菌及铜绿假单胞菌最多,细菌耐药率高,混合感染多见,MRSA、真菌感染亦不容忽视。经验性治疗要广覆盖、重拳猛击,尽早根据细菌培养和药敏目标性治疗。
