摘要
目的:为探讨血管性痴呆病变部位和血流动力学改变及其相关性。方法:采用头颅CT 影像技术及经颅多普勒(TCD) 技术对38 例患者分别进行两项检测。结果:(1) 头颅CT 示病变多在基底节,丘脑多发梗塞及左额、左额顶大面积梗塞,大多合并脑萎缩。(2)TCD 示以大脑中、前动脉异常率为高,表现峰值平均血流速度( Vm) 下降,搏动指数(PI) 增高。结论:病变多位于基底节、丘脑处,其相应的脑血流也减少,说明大脑中动脉。
Objective:To investigate the pathological changes position and hemodynamic changes in vascular dementia and to probe their correlation.Methods:Cranial computer tomography(CT)and transcranial Doppler(TCD)were performed in 38 patients.Results:(1)CT showed that most of the pathological changes were multiinfarct at basal ganglia and thalamus and infarct in large area at the left frontal lobe and the left frontal and parietal lobe,the majority of which were accompanied by cerebral atrophy.(2)TCD showed that the abnormal rate of cerebral median and vertebral artery was higher than that of other areas,which was demonstrated by the decrease in Vm and the increase in PI.Conclusions:Pathological changes mainly occur at basal ganglia and thalamus with cerebral blood flow decreased correspondingly,the hemodynamic changes in cerebral median and vertebral artery are pathological bases for video changes in vascular dementia.
