血管溶通术治疗股骨头缺血性坏死

Treatment of ischemic necrosis of femoral head with thrombolytic therapy

目的：探讨治疗股骨头缺血性坏死的有效方法。方法：对４６ 例股骨头缺血性坏死患者（６７ 个关节） 采用选择性动脉插管至旋股内、外动脉，直接推注溶栓、扩张血管药物。结果：疼痛消失１７ 个关节（ 占２５ ％ ） ，明显减轻２１ 个关节（ 占３１ ％ ） ，减轻２９ 个关节（ 占４４ ％ ） ；关节功能１９ 个关节恢复正常（ 占２８ ％ ） ，４３ 个关节有不同程度改善（ 占６４ ％ ） ，５ 个关节无变化（ 占８ ％ ） 。结论：该疗法可使坏死的股骨头重新获得供血，从而缓解关节疼痛，改善关节功能，达到死骨修复，不失为一种安全有效的治疗方法。

Objective:To discuss the effective treatment method of ischemic necrosis of femoral head.Methods:Thrombolytic and angiectatic remedies were directly infused to 46 cases of ischemic necrosis of femoral head(67 joints)by selective catheterization of lateral femoral circumflex arteries.Results:Pain of 17 joints (25%)was resolved,21 joints(31%)was clearly alleviated,29 joints (44%)was commonly eased.Joint functions of 19 joints(28%)returned to normal,43 joints(64%)improved in different degrees,5 joints(8%)didnt take a turn for the better.Conclusions:This method can impel the necrotic femoral heads regain blood supply,and thereby relieve joint pain,improve joint function,renovate sequestra.Its a safe and effective method for ischemic necrosis of femoral head.
