抓握式接骨板治疗肋骨骨折71例分析

摘要 目的探讨抓握式接骨板治疗肋骨骨折的手术要点;对比镍钛记忆合金及普通钛合金抓握式接骨板的优劣。方法对71例肋骨骨折患者的临床资料进行回顾性分析与总结。结果 71例中40例实施肋骨切开复位镍钛记忆合金抓握式接骨板内固定术,31例实施普通钛合金抓握式接骨板内固定术。术后胸痛均明显减轻;胸廓畸形均得以满意矫治,连枷胸症状消失,呼吸较术前明显改善。单切口镍钛记忆合金板最大固定范围8根肋骨;普通钛合金板5根肋骨。术后随访均达骨性愈合,无接骨板完全松动、脱落现象。结论两者均可有效的改善骨折所致胸部症状,提高患者生活质量,促进骨折愈合。手术安全可靠。镍钛记忆合金抓握式接骨板完成固定时所需操作空间较小;操作简单;单切口能完成更多的骨折固定。更符合临床需求。 Objective To study the key points of operative procedure of treating fractured ribs with portable alloy bone-juncture plate and to compare the advantages and disadvantages of the portable nickle-titanium memory alloy bone-juncture plate and the ordinary portable titanium alloy bone-juncture plate.Methods The clinical data of 71 patients with fractured ribs were retrospectively analysed and summarized.Results Among the 71 cases,40 patients were treated by open reduction and internal fixation with portable nickle-titanium memory alloy bone-juncture plate,while 31 patients were treated by internal fixation with ordinary portable titanium alloy bone-juncture plate.All patients felt that the chest pains were obviously relieved,their chest malformation was corrected satisfactorily,conditions of flail chest disppeared and the respiration function got much better than before the operations.The maximum fixation range of single incision with portable nickle-titanium memory alloy bone-juncture plate was 8 ribs,while the maximum fixation range with ordinary portable titanium alloy bone-juncture plate is 5 ribs.All cases obtained bone union after the operation;no bone plate was completely loosened or dislocated.Conclusion Both methods proved not only effective to improve the chest conditions caused by rib fracture but also very helpful to the quicker healing of rib fracture,and patients' life got improved.Both methods are sucure and reliable to treat rib fracture.Internal fixation with portable nickle-titanium memory alloy bone-juncture plate needs less space to conduct the operation and it is a simple way to treat rib fracture,and more fractured ribs can be fixed with mono-incision,thus it meets clinical needs better.
