肺小细胞癌皮肤转移1例

A Case of Skin Metastasis Due to Small Cell Cancer of Lung

患者男,69岁。背部偏左侧出现肿物半年,破溃半月。皮肤科情况:背部偏左侧可见一2.0cm×10.0cm×8.0cm大肿物,质硬,表面破溃,左侧腋下淋巴结肿大,如鸽蛋大。皮损组织病理示:胶原纤维束间见密集肿瘤样细胞浸润,此种细胞体积较小,呈圆形,胞浆少,并见个别异形细胞。免疫病理示:肿瘤细胞LCA阴性,CgA阳性,CD56阳性,Syn阳性,TTF1阳性,AE1/AE3表皮阳性,肿瘤细胞胞浆内见点状阳性,CK20阴性。诊断:肺小细胞癌皮肤转移。CT检查提示肺部和胰腺有占位性病变,行皮肤肿瘤切除术,术后患者消瘦、纳差,1个月后死亡。

A 69-year-old male presented with a mass on his left back for six months and the lump had ulcerated for half a month. Physical examination showed one mass on patient＇s left back about 2.0cm ×10.0cm ×8. 0cm in size, firm and ruptured on the surface. Lymph node in his left axillary was as large as pigeon egg. Histopa- thology showed dense tumor-like cells in the collagen bundles. The cells were small-sized, round, with less cytoplasm, and several showed heteromorphism. Immunopathology revealed that CgA, CD56, Syn, and TFFI were positive, AE1/AE3 was positive on epidermis and focal cytoplasm positive on tumor cells, besides LCA and CK20 were negative. Diagnosis： Skin metastasis due to small cell cancer of lung. Further, CT found that there were occupying lesions in patient＇s lung and pancreas. After skin tumor resccting, the patient was leptystic and anorexia, then died after one month.
