颌骨大型囊性病变相关牙的保存治疗

Conservative treatment of the teeth involved by large cystic lesion in jaws

目的:研究保存颌骨大型囊性病变相关牙的临床效果。方法:对22例颌骨囊性病变患者的102颗相关受累牙术前行根管治疗。手术彻底刮除颌骨病变组织后,用电钻将暴露于骨腔中的根尖磨除2～3mm。术后随访7～28个月,检查经治牙的松动、咬合、牙根吸收以及骨质修复情况。结果:22例患者颌骨囊性病变均无复发。102颗经治牙中,98颗成功保留,均能正常行使咀嚼功能;4颗因松动加剧于术后3个月拔除。结论:颌骨大型囊性病变相关牙,经术前根管治疗联合术中根尖切除可予有效保存,不增加囊性病变的复发率。

PURPOSE： To study the clinical effect of preserving the teeth involved by large cystic lesion in jaws.METHODS： During curettage of the cystic lesion in the jaws,apicectomy was performed on 102 involved teeth which had received complete root canal therapy.All the patients were evaluated after operation with a follow-up of 7-28 months.RESULTS： Recurrence was not observed and a total of 98 teeth were successfully preserved,4 teeth were extracted due to severe loosening.CONCLUSION： The teeth involved by large cystic lesion in jaws could be successfully preserved by root canal therapy before operation and apicectomy during curettage,with no increase of postoperative recurrence.
