PTEN蛋白在肺癌中表达的定量分析及临床意义 被引量:10

摘要 目的:定量分析PTEN蛋白在肺癌、肺良性病变及正常肺组织中的表达特点及其临床意义。方法:用免疫组化SP法检测肺癌组织、肺良性病变和正常肺组织中PTEN蛋白的表达,用LeicaQ500MC图像分析系统对PTEN蛋白表达的阳性强度进行定量测试。结果:PTEN在正常肺组织和肺良性病变中表达的阳性单位(PU值)呈正态分布,在肺癌中呈正偏态分布,三者间的表达差异具有统计学意义(P=0.000);肺癌组织中PTEN的表达显著低于肺良性病变和正常肺组织;肺的鳞癌、腺癌、大细胞癌和小细胞癌中PTEN蛋白表达的PU值基本相同,差异无统计学意义(P=0.789);肺癌中PTEN的表达与肿瘤的大体类型有关(P=0.023),周围型肺癌PTEN蛋白表达的PU值明显低于中央型肺癌;有淋巴结转移的肺癌组织PTEN的PU值(M=8.68)大于无淋巴结转移的肺癌组织(M=5.47,Z=-0.894,P=0.058);肺癌中PTEN的表达强度与患者年龄、性别、吸烟史、癌组织分化程度及pTNM分期均无关(P>0.05),不同肺良性病变(肺大泡、支气管扩张和肺结核)PTEN的表达差异无统计学意义(P=0.889)。结论:肺癌组织PTEN蛋白的表达明显低于非肺癌组织,且呈正偏态分布;周围型肺癌中PTEN的表达低于中央型肺癌。PTEN蛋白的表达有助于肺癌与正常肺组织及不同大体类型肺癌的区别。 Objective To investigate the expression and clinical significance of PTEN protein in lung cancer. Methods The expressions of PTEN protein in lung cancer, benign pulmonary disease, and normal lung tissue were detected by immunohistoehemical SP assay, and were analyzed quantitatively by a Leica Q5OOMC image analysis system. The single positive intensity (positive unit, PU) was used to quantitatively analyze the expression of PTEN protein. Results The PU of the expression of PTEN protein was normally distributed in both normal lung tissue and benign pulmonary diseases, and was positively skew distributed in lung cancer. The expressions of PTEN protein in lung cancer was significantly lower than those in benign pulmonary diseases and normal lung tissue (P = 0.000). There were no significant differences among the PU values of PTEN protein in squamous carcinoma, adenoeareinoma,large cell carcinoma and small cell carcinoma (P = 0.789). The PU values of PTEN protein in peripheral lung cancer was significantly lower than that in central-type lung cancer(P = 0.023 ), that in lung cancer with lymph node metastases (M = 8.68 ) was higher than that in lung cancer without lymph node metastases (M = 5.47, Z = -0.894, P = 0.058). The intensity of PTEN protein expression was not associated with the patients' gender, age, smoking habit, differentiation of the tumor and pTMN stage (P 〉 0.05 ). There were no significant differences among the expressions of PTEN protein in lung bullae, bronchiectasis, and tuberculosis (P = 0.889). Conclusions The expression of PTEN protein in lung cancer is obviously lower than that in normal lung tissue and benign pulmonary diseases; Expression of PTEN protein is a potential valuable biomarker for differentiating lung cancer,benign pulmonary disease, and normal lung tissue.
