摘要
过去30多年里,关于血压与心血管事件之间的J型曲线一直都是备受关注的话题。众所周知,高血压是心血管疾病最重要的危险因素之一,积极有效的降压治疗可以显著降低心血管事件发生的风险。然而却有大量研究发现,舒张压的降低,也伴随着冠心病和其他不利后果的风险增高。这种效应在接受治疗的冠心病患者中更加显著。因此,强化降压的理念受到了巨大的冲击,而降压目标究竟该如何选择,再一次成为了内科医生们所面临的难题。对高血压中的J型曲线的回顾与分析会为我们制定科学合理的降压策略提供依据。
During the past 30 years,the J-curve between blood pressure and cardiovascular events has been a subject of much concern.As we know,hypertension is one of the most important risk factors in coronary artery disease;a risk that could be significantly lowered by active and effective antihypertensive treatments.However,as observed in numerous studies,we have noticed that lowering the diastolic pressure has also brought greater risk of coronary heart disease and adverse outcomes.This effect is even more pronounced in patients with underlying coronary artery disease.As a result aggressive blood pressure-lowering has been impacted,and physicians are once again stuck for what to choose as a target level.Reviews and analyses of the J-curve in hypertension provide a basis for rational medication.
