糖尿病肾病与血清内脂素、C反应蛋白的关系 被引量:3

摘要 目的探讨糖尿病肾病(DN)与血清内脂素、C反应蛋白(CRP)的关系。方法选择97例2型糖尿病患者,据尿白蛋白排泄率(UAER)分为3组:正常蛋白尿组(UAER〈20μg/min)35例、微量蛋白尿组(UAER 20~200μg/min)30例、大量蛋白尿组(UAER〉200μg/min)32例,另选健康体检者35例为对照组。采用ELISA法检测血清内脂素、免疫比浊法测定CRP,同时测定各组空腹血糖(FPG)、糖化血红蛋白(HbA1 c)、TG、TC、LDL-C、HDL-C、尿素氮(BUN)、血肌酐(Scr)等。结果糖尿病组患者血清内脂素、CRP水平明显高于对照组(P均〈0.05),且随着UAER的增加,血清内脂素、CRP水平逐渐升高(P均〈0.01)。相关性分析显示,血清内脂素、CRP均与糖尿病病程、收缩压、HbA1 c、FPG、TG、TC、LDL-C、BUN、Scr、UAER呈不同程度正相关(r内脂素=0.943、0.699、0.511、0.371、0.362、0.398、0.485、0.623、0.832、0.890;rCRP=0.777、0.701、0.416、0.327、0.307、0.449、0.611、0.817、0.893,P均〈0.01),与HDL-C呈负相关(r=-0.617、-0.546,P均〈0.01),且两者互呈正相关(r=0.940,P〈0.01)。结论血清内脂素、CRP水平随着DN的发展而逐渐升高,两者可能参与了DN的进展。
