双吻合器在低位直肠癌保肛术的应用

The Application of double stapler technique for anus-preserved in low rectal cancer

目的探讨低位直肠癌保肛术的可行性及全直肠系膜切除加双吻合器技术的应用效果。方法回顾性分析我院2000~2010年118例直肠癌患者应用全直肠系膜切除加双吻合器行低位直肠前切除术的临床资料。结果本组无手术死亡病例,吻合口漏4例,发生率为3.38%;吻合口狭窄12例,发生率为10.17%。术后复发8例,复发率为6.7%,本组无排尿及性功能障碍。结论全直肠系膜切除和双吻合器技术行低位直肠癌保肛术是一种安全有效的手术方式,可降低术后局部复发率,明显提高保肛成功率。

Objective To discuss the feasibility of anal preserving for low rectal cancer and the effect of total mesorectal excision（TME） with double stapling technique（DST）.Methods 118 Patients with low rectal cancer from 2000 to 2010 were treated by low anterior resection of the rectal carcinoma in which TME and DST were used.Results No operative death was noted.Anastomotic fistula and anastomotic stenosis were found in 4 patients（3.38%） and 12 patients（10.17%）,respectively.Local recurrence was noted in eight patients（6.7%）.No dysuria and sexual dysfunction occurred.Conclusion TME with DST is a safe and effective operation method to reserving anus operation in lower rectal cancer.It can reduce postoperative local recurrence rate and obviously improve the anus success rate.
