无创颅内压监测在双额叶脑挫裂伤中的应用 被引量:8

摘要 目的探讨无创颅内压(NICP)监测在双额叶脑挫裂伤中的应用。方法对58例双额叶脑挫裂伤患者给予NICP监测,16例因病情发生变化颅内压超过500mmH20(1mmH:0=0.0098kPa),行颅内血肿清除术+去骨瓣减压术,其余42例患者经脱水等处理后颅内压下降。根据格拉斯哥预后量表(GOS)评分进行分级,Ⅰ级28例,Ⅱ级13例,Ⅲ级12例,Ⅳ级5例。结果标准的N2波宽约为70m8,在刺激后的150ms即可出现,N2波形状典型,潜伏期及波宽恒定。脑外伤患者由于颅内压升高,在视觉诱发电位中的潜伏期有明显的变化,N2波的潜伏期长短和颅内压变化有明显的直线相关性,相关系数为0.9。当N2波潜伏期大于90ms时,就会有颅内压升高。结论对脑外伤患者进行NICP监测,能够迅速、客观、准确地实时了解颅内压的动态变化,既有利于迟发性颅内血肿的早期诊断,又可更准确地判断预后、指导治疗、提高疗效。
