术前输注氟比洛芬酯对髋关节置换术患者血浆IL-6浓度的影响

Effect of Intravenous Injection of Flurbiprofen Axetil Preoperative on Plasma Interleukin-6 in Patients undergoing Total Hip Replacement

目的:探讨术前静脉输注氟比洛芬酯注射液对髋关节置换术患者血浆细胞因子IL-6的影响。方法:选择髋关节置换手术患者40例,ASAⅠ～Ⅱ级,随机分为生理盐水组(n=20)和氟比洛芬酯组(n=20)。两组患者常规腰硬联合麻醉,实验组于手术开始前静脉予以氟比洛芬酯注射液(10mg/ml)1mg/kg,对照组予以生理盐水0.1ml/kg。记录麻醉前、麻醉后30分钟及手术结束时两组患者心率,收缩压,舒张压。麻醉后30分钟并对患者进行VAS评分。于手术切皮前(T_1)、手术结束后(T_2)及术后24h(T_3)采集外周静脉血,测定血浆IL-6含量。结果:两组患者VAS评分及各时段心率,收缩压,舒张压组间比较无明显变化(P>0.05)。T_2,T_3时实验组IL-6水平较对照组明显降低(P<0.01)。结论:氟比洛芬酯能降低髋关节置换手术患者IL-6水平,能产生良好的减轻炎症和免疫调节作用。

Objective：To evaluate the effect of intravenous injection offlurbiprofen axetil（FA）preoperative on expression of plasma IL-6 in patients undergoing total hip replacement.Methods：forty selective total hip replacement patients were randomly divided into 2 groups with 20 cases each. FA（F）group,intravenous injection of FA （10mg/ml）1mg/kg 5 minutes before surgical incision. Normal Sodium （NS） （N） group, intravenous injection NS 0. lml/kg.record heart rate, systolic pressure, diastolic pressure of patients in both groups before anesthesia and 30mns after anesthesia and after operation. Pain scales was assessed by visual analog scales（VAS）at 30mins after anesthesia. Peripheral venous blood samples were respectively taken before anesthesia（Tt）,at the end of operation（T2）and at 24th hour after oparation（T3）,and detected the level of IL-6 in the plasma. Results：VAS and heart rate, systolic pressure, diastolic pressure of patients in both groups was not significantly difference in different times（P〉0.05）.The level of IL-6 in F group was significantly lower than that in N group at T2 and T3（P〈0.05）.Conelnsion： Intravenous injection of FA before total hip replacement can relieve inflammatory reaction and improve immunologic function.
