查错奖励法应用于输液流程安全管理效果研究

Study on the effect of troubleshooting incentive act used in transfusion safety management process.

目的:将"查错奖励法"应用于输液流程中,强化安全管理,保障患者输液安全。方法:2009年5月～2010年12月,对门诊输液区所有输液流程均采用"查错奖励法"促使查对制度的落实,即将输液流程分为接待电脑班-配药班-注射班三个环节,每班针对上一班存在的未发现的错误进行查对,使存在的错误充分暴露。同时尽量让本环节不出错。对查出的错误予以一项奖励工作积分10～100分。并由护理安全小组对错误及时进行科学分析与流程改进,预防由错误演变成差错事故。结果:2009年5月～2010年12月,我院输液区输液共280 926人次,其中3岁以下小儿12 435人次。通过"查错奖励法"共查出错误4 528处。统计分析后改进输液相关流程10余项,最终差错事故发生率为0。结论:"查错奖励法"应用于输液流程安全管理,对预防差错事故的发生具有重要作用。

Objective： The Troubleshooting Incentive Act infusion process used to strengthen safety management,protect the patient transfusion safety.Methods： In May 2009 to December 2010,on the outpatient infusion area were used for all transfusion processes,Troubleshooting to promote the implementation of the system check,the computer will soon infusion process is divided into reception classes-dispensing Class-Class III injection a link,the class does not exist for the class of error checking to check out,so there was an error fully exposed.At the same time try to make this session is not wrong.The error of detected points to the work of an award 10-100 points.Care and safety team on the error by timely scientific analysis and process improvement,prevention of errors by the error evolved into an accident.Results： From May 2009 to December 2010,our hospital a total of 280,926 passengers infusion infusion area,including 12,435 children under the age of 3 times.Through the troubleshooting Incentive Act error 4528 were identified.Improved statistical analysis of transfusion-related processes more than 10,the final error accident rate is 0.Conclusion： Troubleshooting Incentive Act applies to transfusion process safety management,accident prevention errors can achieve satisfactory results.
