系统性红斑狼疮患者心脏损害临床分析

Clinical analysis on heart involvement in patients with systemic lupus erythematosus

目的探讨伴发心脏损害的系统性红斑狼疮患者临床特征,分析系统性红斑狼疮活动度及抗心磷脂抗体和心脏病变的相关度。方法回顾性分析2008～2010年收治的系统性红斑狼疮患者230例,其中125例伴发心脏病变。结果心包炎患者65例(28.3%),瓣膜病变患者42例(18.3%),心肌病变6例(2.6%),心律失常10例(4.3%)。伴发心包炎患者疾病活动度和无伴发心包炎患者差异有统计学意义(P<0.05),伴发瓣膜病变患者抗心磷脂抗体和无伴发瓣膜病变患者差异有统计学意义(P<0.05)。结论系统性红斑狼疮伴发心脏病变中,以心包炎最为常见,心脏损害和疾病活动度,抗心磷脂抗体相关。

Objective To explore the clinical features of heart involvement in patients with systemic lupus erythematosus,and to analyze the correlation between activity of systemic lupus erythematosus and anticardliolipin antibodies with cardiopathy.Methods Among 230 patients with systemic lupus erythematosus admitted in this hospital during 2008-2010,125 of them complicated with cardiopathy.Results Sixty five patients were complicated with pericarditis,42 patients complicated with valval lesions,6 patients with cardiomyopathy,and 10 patients with arrhythmia.There was statistically significant difference in activity of disease between patients complicated with pericarditis and those patients without pericarditis（P0.05）.The difference in presence of anticardiolipin antibodies in patients with valval lesions and those patients without valval lesions was statistically significant（P0.05）.Conclusion Among heart involvement associated with systemic lupus erythematosus,the most common complication is pericarditis.The activity of disease and heart involvement are closely related to the presence of anticardiolipin antibodies.
