人工膝关节置换术围手术期感染的预防与控制

Infection occurred in artificially knee arthroplasty during perioperative period:analysis of prevention and control

目的分析人工全膝关节置换术围手术期感染的发生原因及其防治措施。方法将医院2002年10月-2010年4月,收治的行人工全膝关节置换术104例患者随机分为观察组和对照组,对照组采用围手术期的常规护理,观察组在对照组的基础上针对有可能发生的感染采取控制易感因素预防性干预,比较两组患者的切口愈合、拆线时间、感染的发生率以及远期疗效。结果观察组患者切口甲级愈合率、拆线时间、感染率,分别为96.2%、(12.6±3.2)d、3.8%均显著优于对照组,差异有统计学意义(P<0.05);远期疗效观察组优30例、良13例、总优良率为82.7%,显著高于对照组55.8%,P<0.05。结论良好的围手术期控制易感因素的预防干预,可减少感染的发生,提高手术治疗的疗效。

OBJECTIVE To analyze the causes and prevention and cure measures for infections in artificially total knee arthroplasty during perioperative period.METHODS A total of 104 cases performed artificial total knee arthroplasty from Oct 2002 to Apr 2010 were randomly divided into trial group and control group,and were treated by conventional perioperative care,the observation group took control risk factor preventive intervention based on the control group with possible infection,the healing of incision,stitches time and infection of the incidence and long-term curative effects of two sets of patients were compared.RESULTS The observation group patients：the incision coexisted pathology rate,the stitches time,infection and the incidence of infection were 96.2%,（12.6±3.2）d and 3.8%,which were significantly higher than that of control group,（P0.05）.long-term follow-up of the observation group optimal were 30 cases in ＂excellence＂ and ＂good＂ in 13,the total excellent and good rate in observation group（82.7%） was significantly higher than that of the control group of 55.8%,（P0.05）.CONCLUSION The excellent preventive interventions for controlling predisposing factors performed during perioperative period can reduce the infection incidence and improve the curative effect.
