上海市72所医院血培养标本送检率调查

Submission rate of blood culture specimens in 72 hospitals in Shanghai

目的了解上海市二甲以上72所医院血培养标本送检率情况。方法上海市医院感染质控中心要求所有医院于2011年3月16日上午,调查3月13-15日出现发热≥38.5℃的全体患者,收集其感染及血培养送检情况。结果 72所医院共调查54 968例患者,发热≥38.5℃的1602例(2.92%),血培养送检患者有604例(38.3%),三级医院留置导管患者血培养送检率35.2%,低于二级医院的40.8%(P=0.003)。结论上海市二甲以上医院血培养送检率低,血流感染诊断依据不足,存在大量漏报。

OBJECTIVE To investigate the rate of blood culture submission in 72 hospitals in Shanghai.METHODS The investigation of blood culture submission rate in 72 hospitals was carried out under the requirement of Shanghai nosocomial infection control center.All of the hospitals retrospectively evaluated the submission rate in the past 3 days on Mar 16th,2011.The data were reported to the center via the network.RESULTS All over,54,968 patients were enrolled in the study,1602（2.92%） cases had a BODYtemperature 38.5 ℃,604（38.3%） of them had taken blood culture for diagnosis.The rate of blood culture submission in fever patients who had a central venous catheter in tertiary hospitals was 35.2%,which was significantly lower than 40.8% in the secondary hospitals（P=0.003）.CONCLUSION The submission rate of blood culture submission in the tertiary hospitals is quite low,it is possible that the blood stream infection is underestimated due to the insufficient diagnostic evidence.
