综合康复疗法治疗2型糖尿病胃轻瘫的临床研究

Clinical study of combined rehabilitation therapy in patients with type 2 diabetic gastroparesis

目的观察综合康复疗法治疗2型糖尿病胃轻瘫的疗效。方法将40例2型糖尿病胃轻瘫(GDP)患者随机分为对照组及治疗组,对照组仅给予口服药物(多潘立酮片10mg口服,每天3次)治疗。治疗组在对照组基础上给予电针、高压氧、有氧锻炼的综合康复疗法。分别于治疗前和治疗后评定两组患者空腹血糖、餐后2小时血糖、血脂、胰岛素抵抗指数、体重指数、消化道症状评分、胃排空时间、自主神经功能异常、胃动素等疗效指标。结果治疗组治疗后的空腹血糖、餐后2小时血糖、糖化血红蛋白、总胆固醇、甘油三酯、低密度脂蛋白胆固醇与治疗前比较明显下降,差异均有统计学意义(t分别=0.93、0.64、1.01、1.27、2.01、1.54,P均<0.05);与对照组治疗后比较,差异也均有统计学意义(t分别=0.98、0.71、1.25、1.79、2.08、1.68,P均<0.05)。治疗组治疗后消化道症状评分、胃排空时间、体重指数与治疗前比较有明显好转,差异均有统计学意义(t分别=0.66、1.58,1.97,P均<0.05)。治疗组治疗后症状评分、胃排空时间、体重指数与对照组治疗后比较有明显改善,差异有统计学意义(t分别=1.39、1.98,2.07,P均<0.05)。结论综合康复疗法治疗2型糖尿病胃轻瘫具有明显疗效。

Objective To observe the effect of combined rehabilitation therapy on the diabetic gastroparesis patients. Methods Forty type 2 diabetic gastroparesis patients were randomly divided into control group and therapy group. Patients in control group took orally only domperidone, 3 times per day. Patients in therapy group get the same drugs and received the combined rehabilitation therapy （electroacupuncture ＋ hyperbaric oxygenation ＋outdoor exercises）.Fasting blood glucose, 2 hours postprandial blood glucose, lipid, insulin resistant index, body mass index, enteron symptom score, curative effect, gastric emptying time, dysautonomia and motilin were compared before and after treatment in all patients Results There were statistic differences in FBG, 2hPBG, HbAlc,TC,TG and LDL before and after treatment in the therapy group（t=0.93,0.64,1.Ol,l.27,2.01,1.54, P〈O.05）. FBG, 2hPBG, HbAlc, TC, TG and LDL in the therapy group had statistic differences compared with that of the control group after treatment （t=0.98,0.71,l.25,1.79,2.08,1.68,P〈0.05）. Enteron symptom score, gastric emptying time and body mass index were decreased after treatment in the therapy group and there were statistic differences compared with those before treatment （t=0.66,1.58,1.97, P〈0.05）and compared with the control group after treatment there were statistic differenees（t=l.39,1.98,2.07, P〈0.05）. Conclusions Combined rehabilitation therapy has significant therapeutic effect on the diabetic gastroparesis patients.
