社区高血压患者心电图明尼苏达编码与最大氧耗的相关性研究 被引量:1

摘要 目的通过对社区高血压病人进行次极量运动试验后最大摄氧量(VO2max)及心电图明尼苏达编码的测定,探讨不同程度的高血压病人的VO2max与心电图明尼苏达编码之间的关系,以发现VO2max的变化对心脏损害的预测价值。方法将所选的社区高血压患者80人按高血压的程度分成轻、中、重三组,健康对照组20人。对受检者进行静息心电图测定并进行次极量运动试验,运动终止即刻测定VO2max,并进行统计学分析。结果对各组年龄、性别、心电图明尼苏达编码数据与VO2max值之间及高血压程度之间进行多元回归分析发现:性别、年龄、左室高电压异常(编码3)、T波异常(编码5)是VO2max异常的独立危险因素(β分别=-0.39、-0.28、-0.19、-0.30,P均<0.05);性别、年龄、ST-T异常(编码4)以及VO2max与高血压程度之间具有独立危险性(β分别=-0.29、-0.24、0.31、-0.82,P均<0.05)。结论高血压病人的最大氧耗量降低与高血压的程度有关,左室劳损的高血压患者最大氧耗量明显降低。所以对高血压病人进行心电图明尼苏达编码及VO2max的测定能较早预测高血压病人的心脏损害程度,为高血压的早期控制及预防心脏损害提供依据。 Objective By testing the maximal oxygen uptake(VO2max) of hypertensive patients after doing extreme exercise and Minnesota coding of the electrocardiogram to determine the connection between VO2max and Minnesota coding of the electrocardiogram for hypertensive patients with different degrees.Methods Divided the 80 hypertensive patients into three groups:light,moderate and severe according to the degree of their blood pressure,and a group with 20 healthy people.Gave the testers the electrocardiogram test and maximal exercise test and immediately determined VO2max as the exercise terminated.Results In multiple logistic regression analysis of gender,age,Minnesota coding of the electrocardiogram,VO2max and the levels of hypertension:gender,age,abnormal left ventricular high voltage(code 3),T wave abnormalities(code 5) were the independent risk factors of VO2max(β=-0.39,-0.28,-0.19,-0.30,P0.05);And there were the independent risk in gender,age,ST-T abnormalities(code 4),VO2max and the levels of hypertension(β=-0.29,-0.24,0.31,-0.82,P0.05).Conclusions The reduction of VO2max in patients has connection with the level of the blood pressure:The VO2max in patients with left ventricular strain decreased significantly.Therefore,taking Minnesota coding of the electrocardiogram and VO2max tests can predict the heart damage in advance,which can provide evidence for the early control and heart damage for hypertensive patients.
