内镜微波治疗胃贲门粘膜撕裂综合征

THERAPY OF MALLORY-WEISS SYNDROME WITH ENDOSCOPIC MICROWAVE

评价胃贲门粘膜撕裂综合征内镜波微治疗的效果。方法：将１９９３ 年１ 月～１９９９ 年１ 月收治的３４例有活动性出血的胃贲门粘膜撕裂综合征病例随机分组，实验组应用内镜微波治疗，对照组经内镜局部用药。结果：内镜微波治疗组一次止血率明显高于局部用药组（Ｐ＜ ０．０１） ，两者对裂伤粘膜愈合的影响无显著性差异。内镜治疗未见远期并发症。结论：内镜微波治疗胃贲门粘膜撕裂综合征治疗效果肯定，方法简便。

Objective:To evaluate the effect of microwave therapy under endoscope for Mallory-Weiss syndrome(MWS).Methods:A prospective study was performed in this project.From Jan.1993 to Jan.1999,34 MWS patients with active bleeding were treated with endoscopic microwave in our department.The patients were divided into 2 groups randomly as followings:the experimental group:17 cases treated by endoscopic microwave therapy;the control group:the other 17 cases treated by endoscopic local injection and sprinkling (1k u of Reptilas).Results:All the cases of the experimental group obtained definite hemostasis after primary treatment.15 cases of the control group got definite hemostasis after single or double therapies,2 cases had to be administered endoscopic microwave therapy on the third time.The effect of the experimental group was significantly better than that of the control group ( P <0.01).There was no statistic difference on the healing course of the mucosal laceration between these two groups ( P >0.3). Conclusions:Endoscopic microwave therapy has definite hemostasis effect,convenient management for treating MWS.The successful application of this method may avioid operative therapy.
