侵入颈内静脉孔的转移性肿瘤切除术

The management of the metastatic carcinoma which invaded jugular foramen on 5 patients with laryngeal or pharyngeal carcinoma

目的：探讨侵入颈内静脉孔转移性肿瘤的外科治疗方法。方法：4例喉癌和1例喉咽癌高位颈淋巴结转移并侵入颈内静脉孔的患者，施行高位颈淋巴结清扫术。结果：5例创口一期愈合，除1例口角左斜及张口困难外，未见其他手术并发症。随访1至5年，4例存活，1例术后3年半死于对测颈转移。结论：高位颈清扫术是治疗侵入颈内静脉孔转移性肿瘤的有效方法，而手术的关键是切除乳突及下颌骨部分升支，扩大乳实下颌间隙及颈内静脉孔。

The experience of cervical metastatic carcinoma management on 5 pa-tients with laryngeal or pharyngeal cancer was reported. There were 5 males,ranging inage from 45 to 71. The surgical procedue included mastoidectomy, partial excission ofmandibula ramus segment, extending jugular foramen, and ligation of jugular voins inhigh level. All patients were followed up from 1 to 5 years.. Result showed that 4 caseswere alive over 3 years (one case died in 3. 5 years after operation because of cancer re-currence),one case is alive over 1. 5 years and still in continue follow-up.
