清肾祛瘀方治疗糖尿病肾病Ⅲ期临床研究

Clinical Study of Qingshen Quyufang on Diabetic Nephropathy Stage Ⅲ

目的观察清肾祛瘀方对糖尿病肾病Ⅲ期的临床疗效。方法将糖尿病肾病Ⅲ期患者180例按随机区组分为治疗组和对照组,每组各90例。2组均给予糖尿病健康教育、饮食控制及适量运动,严格控制血糖、血压,治疗组在此基础上加用清肾祛瘀方,对照组在此基础上加用氯沙坦,治疗12周。观察2组疗效及治疗前后尿白蛋白排泄率(UAER)、空腹血糖(FPG)、血脂、血液流变学、糖化血红蛋白(HbA1c)等指标变化情况。结果治疗组总有效率(91.1%)优于对照组(64.4%),差异有统计学意义(P<0.05)。治疗组UAER、FPG、HbA1c、血脂、血液流变学等指标比对照组均明显降低(P<0.05)。结论清肾祛瘀方可以明显改善糖尿病肾病Ⅲ期患者的临床症状,降低早期糖尿病肾病患者UAER,改善血脂紊乱,降低血液黏稠度,提高糖尿病肾病患者的生存质量。

Objective To observe the clinical efficacy of Qingshen Quyufang on diabetic nephropathy（DN） stageⅢ.Methods One hundred and eighty cases of DN stageⅢwere randomly divided into treatment group and control group,90 cases in each group.Two groups were given health education of diabetes,diet control and regular exercise,strict control of blood glucose and blood pressure.The treatment group was given Qingshen Quyufang and the control group was given losartan additionally for 12 weeks.Clinical efficacy and urinary albumin excretion rate（UAER）,fasting plasma glucose（FPG）,blood lipid, hemorrheology indices,glycated hemoglobin（HbA1c）before and after treatment were observed.Results The total effective rate of the treatment group（91.1%）was better than the control group（64.4%）,with statistically significant difference（P〈0.05）.UAER,FPG,HbA1c,lipids and hemorrheology indices of the treatment group were significantly lower than the control group（P〈0.05）.Conclusion Qingshen Quyufang can obviously improve the clinical symptoms of DN stageⅢpatients,reduce UAER of early DN patients, alliviate blood lipid disorder and blood stickiness,and improve the quality of life of DN patients.
