500例冠心病患者中医体质分布特点及其与证候的关系

Distribution Characteristics of TCM Constitutional Types in 500 Patients with Coronary Heart Disease and Relationships between These Characteristics and Syndromes

目的调查冠心病患者中医体质类型,研究体质与证候的关系,为冠心病的中医防治提供依据。方法对500例冠心病患者采用中医体质问卷进行中医体质调查,并按照王琦分型法分析诊断体质,以进行体质和证候的相关分析。结果患者病理体质中,瘀血质出现频率最高,其他依次为气虚质、痰湿质、阳虚质、湿热质、阴虚质、气郁质、平和质、特禀质;心血瘀阻证、痰阻心脉证和心阴亏虚证等证候为冠心病患者的主要证候特点。瘀血质与心血瘀阻证、痰阻心脉证和心阳亏虚证显著相关,气虚质与心阴亏虚证、心气亏虚证和心阳亏虚证显著相关,痰湿质与痰阻心脉证和心阳亏虚证显著相关,阳虚质与寒滞心脉证、心气亏虚证和心阳亏虚证显著相关,阴虚质与心阴亏虚证、心气亏虚证显著相关,气郁质与气滞心脉证显著相关。结论瘀血质、气虚质和痰湿质为冠心病患者的常见体质类型,冠心病患者的体质类型与中医证候相密切相关。

Objective To explore the characteristics of TCM constitution of patients with coronary heart disease（CHD）and study the relation between constitutions and syndromes,to provide basis for preventing and treating CHD.Methods Constitution questionnaires and coronary angiography were used to survey 500 patients with CHD.Constitutions were analyzed and diagnosed based on WANG Qi＇s Constitutional Classification.The correlation between constitution and syndrome was analyzed.Results In 500 CHD cases,the pathological constitutional types are mostly blood stasis constitution,followed by Qi deficiency constitution,phlegm-dampness constitution,Yang deficiency constitution,dampness-heat constitution,Yin deficiency constitution,Qi stagnation constitution in sequence.The syndrome types are mostly heart blood stasis syndrome,heart collaterals obstruction due to phlegm accumulation syndrome and heart-Yin deficiency syndrome.Blood stasis constitution was correlated significantly to heart blood stasis syndrome,heart collaterals obstruction due to phlegm-accumulation syndrome, heart-Yang deficiency syndrome.Qi deficiency constitution was correlated significantly to heart-Yin deficiency syndrome,heart-Qi deficiency syndrome and heart-Yin deficiency syndrome.Phlegmdampness constitution was correlated significantly to heart collaterals obstruction due to phlegm accumulation syndrome and heart-Yang deficiency syndrome.Yang deficiency constitution was correlated significantly to heart collaterals obstruction due to cold stagnation,heart-Qi deficiency syndrome and heart-Yin deficiency syndrome.Yin-deficiency constitution was correlated significantly to heart-Yin deficiency syndrome and heart-Qi deficiency syndrome.Qi stagnation constitution was correlated significantly to heart collaterals obstruction due to Qi stagnation syndrome.Conclusion The most common constitutions are blood stasis constitution,Qi deficiency constitution and phlegmdampness constitution.There is close correlation between TCM constitution and syndrome of patients with CHD.
