呼吸机无创双水平正压通气（BIPAP）治疗COPD的临床评价

目的：探讨呼吸机无创双水平正压通气（BIPAP）在慢性阻塞性肺疾病（COPD）伴II型呼吸衰竭中的临床评价.方法：对入选26例病例使用无创双水平正压呼吸机面罩辅助通气,动态观察NIPPV治疗前和治疗后2h,24h及3d动脉血气及治疗后病人的转归.结果：26例患者中23例无创呼吸机治疗前后,PaCO2明显降低,PaO2明显升高,均脱机成功出院.结论：无创双水平气道正压机械通气辅助治疗COPD合并II型呼吸衰竭具有肯定的疗效.

Objective To investigate the clinic effect of the bilevel positive airway pressure （BIPAP）assisted ventilation on patients of chronic obstructive pulmonary disease（COPD） with type II repiratory failure. Method26 patients with acute exacerbation of COPD with and severe respiratory failure were enrolled. Subjects received assisted ventilation by bi--level noninvasive positive ventilator. The change of arterial blood gas, conscious statewere investigated. Result NIPPVincreased PaO2 significantly and declined PaCO2 significantly.. Twenty--three patients weaned from the ventilator and discharged successfully. ConclusionThebi--level positive airway pressure （BIPAP）assisted ventilation is certainly effective to chronic obstructive pulmonary disease （COPD） with type II respiratory failure.
