重型颅脑损伤患者院内真菌感染原因探讨及防治

Patients with severe brain injury causes of nosocomial fungal infectious and prevention

目的：探讨重型颅脑损伤患者院内深部真菌感染的因素和防治措施。方法：回顾性分析110例重型颅脑损伤患者临床资料，其中继发深部真菌感染患者62例（感染组），无深部真菌感染的48例（对照组）进行比较。结果：重型颅脑损伤患者深部真菌感染的原因以基础疾病严重、广谱抗生素的应用、侵袭性操作、机械通气下亚低温治疗等是继发深部真菌感染的主要因素。结论：重型颅脑损伤继发深部真菌感染的发生有其特点，应重视清除诱发因素，做到早诊断、早治疗、预防用药、用高效、低毒的抗真菌药治疗。

Objective To investigate the hospital with severe head injury patients with deep fungal infection of the factors and control measures. Methods Retrospective analysis of 110 patients with clinical data of patients with severe traumatic brain injury, including deep fungal infection secondary to 62 patients （infection group）, no deep fungal infection in 48 patients （control group）. Results in patients with severe brain injury because of deep fungal infection to a serious underlying diseases, the application of broad--spectrum antibiotics, invaaive operation, mechanical ventilation, hypothermia and other buildings is a secondary fungal infection of the main factors. Conclusion severe traumatic brain injury secondary to invasive fungal infection has its own characteristics, predisposing factors should pay attention to clear, so early diagnosis and treatment, preventive medication, with high efficiency, low toxicity of the antifungal treatment.
