勃起障碍评定量表的编制与测试

Development of Erectile Dysfunction Rating Scale

目的：编制适于评定男性勃起障碍症状及临床标准化诊断的性功能量表。方法：根据临床需要，结合ＥＤ症状学特点，通过对２７９ 例受试者的检验（１４６ 例ＥＤ 患者和１３３ 例正常对照） ，从９０ 道未经临床验证的题项中筛选出２２ 个最终题项，编制成“勃起障碍评定量表”（ＥＤＲＳ） 。结果：ＥＤＲＳ各项目及总分对照组与ＥＤ组差别显著（ Ｐ＜ ０ ．０００１） 。量表重测信度为０－８５７，分半信度为０－９７３ ，α系数为０－９６。因素分析所获三因子（ 性唤起、性活动、性满意） 可解释６５－５５％ 的变异，与ＤＳＭ－ Ⅳ中ＥＤ的诊断标准存在着对应关系。对ＥＤ患者与正常被试以分界值５４（２０ ～４９ 岁组）及５９（ ≥５０ 岁组） 作诊断划界分，临床诊断试验结果表明总灵敏度为９５－８９ ％ ，总特异度为９５－４９％ ，总准确度为９５－７０ ％ 。并且初步划分出２０ ～４９ 岁组患者不同障碍程度ＥＤＲＳ值的分布范围。结论：ＥＤＲＳ的信度、效度初步达到心理测量学要求。

To obtain sexual dysfunction scale for use in clinical standardization diagnosis and erectile dysfunction symptom assessment.Method:The Erectile Dysfunction Rating Scale(EDRS) was developed according to clinical need combining with semeiology features by testing 279 cases (146 ED patients and 133 normal control subjects). The scale consisting of 22 final items was from 90 items without clinical validation.Results:Significant difference in total score and each item scores were found between ED group and normal group. The EDRS indicated satisfactory reliability, as revealed by re-retest reliability (r=0.857), split-half reliability(r=0.9730), and alpha coefficient( α=0.96). Factor analysis resulted in three-factor model accounting for 65.55% of total variance. The three factors identified were sexual arousal, sexual activity and sexual satisfactory. They corresponded to ED diagnosis criterion in DSM-Ⅳ. The cut-off score of 20-49 years group and older than 50 years goup were 54 and 59 respectively. The clinical testing result indicated the validity of EDRS was good, as shown by total sensitivity (95.89%), total specificity (95.49%), total accuracy (95.70%). Distributiong range in EDRS scores was marked off among the differ disorder degree patients of 20-49 group.Conclusion:The EDRS attained satisfactory psychometric properties and pragmatic values.
