Evaluation for results ofMadigan′sprostatectom y using pressure-flow studies

目的：评价Ｍａｄｉｇａｎ 前列腺切除术（ＭＰＣ）的疗效。方法：对２１ 例接受ＭＰＣ的前列腺增生症（ＢＰＨ）患者术前及术后分别进行症状评分及压力－流率测定，客观评价其手术疗效，并与２２ 例耻骨上经膀胱前列腺切除术（ＳＰＰＣ）患者结果对比。结果：ＭＰＣ术后症状评分明显降低、各项压力－流率测定参数及诊断梗阻的百分率明显改善，但改善程度不及ＳＰＰＣ，且残留梗阻率高于ＳＰＰＣ；ＭＰＣ的手术并发症、出血量、血尿时间、导尿管引流时间及住院时间均低于ＳＰＰＣ。结论：ＭＰＣ能够有效地解除膀胱出口梗阻，具有一些ＳＰＰＣ所不及的优点，并发症较少。

Purpose:To evaluate the results ofMadigan′s prostatectom y (MPC).Methods:A totalof43 patientsw ith benign prostatic hyperplasia (BPH) w ere divided into 2 groups,21 patients underw entMPCw hile 22 did suprapubic prostatectom y (SPPC).The pressure-flow analyses w ere perform ed am ong allpatients before and 6 m onths afteroperation,and L-PURR,URA,A-G nom a- gram ,AG num ber and Atheo w ere used to diagnose the bladder outletobstruction (BOO).Results: 21 patients w ith MPCw ere ofa significantim provem entin allparam eters used to diagnose and to grade BOO and the internationalprostate sym ptom score (I-PSS) afteroperation,the sam e changes w ere show ed in 22 patients w ith SPPC.The incidence of residualobstruction in MPCw as higher than thatofSPPC.Theadvantagesand the com plications w ere com pared betw een 2 groups,MPCis w ith the advantages ofabsence of postoperative hem aturia,avoidance ofpost-catheter structure,a shortperiod ofhospitalization and antegrade ejaculation.Conclusions:MPCis effectively able to re- lieve BOO,and w ith som e advantages to SPPC;butthe ability ofrelieving BOOofMPCseem s to be w orse than that ofSPPC.
