血浆N-末端脑利钠肽在老年急性呼吸困难诊断中的价值分析被引量：1

摘要目的:研究血浆N-末端脑利钠肽(NT-proBNP)水平检测在临床老年急性呼吸困难诊断中的应用价值。方法:选取以急性呼吸困难为主诉住院年龄>60岁的患者,剔除由外伤、心包填塞和急性冠状动脉综合征所致的呼吸困难患者,共185例,利用胶体金法对其进行NT-proBNP检测,观察比较心源性呼吸困难(主要指CHF患者)和非心源性呼吸困难患者血浆中NT-proBNP的浓度。结果:在112例(61%)CHF引起的呼吸困难患者中,NT-proBNP为(1 876.63±423.27)ng/L,明显高于73例(39%)非CHF引起的呼吸困难患者,NT-proBNP为(46.82±8.25)ng/L。心功能Ⅰ或Ⅱ级组、Ⅲ级组、Ⅳ级组NT-proBNP分别为(451.26±53.17)ng/L、(1 253.24±126.35)ng/L、(1 978.13±367.27)ng/L,各组间NT-proBNP差异有统计学意义(P<0.01)。结论:对于老年急性呼吸困难患者,NT-proBNP水平的检测用于诊断或者排除CHF十分有价值,可以作为诊断CHF的独立性指标,还可作为心功能分级的参考指标之一,值得临床推广。
