常规静息心电图ST-T改变对老年冠心病患者的诊断价值

The diagnostic value of routine resting ECG ST-T changes on coronary heart disease of old age

目的探讨老年患者静息心电图(ECG)慢性ST-T改变对冠心病的诊断价。方法对215例需接受冠脉造影的疑诊冠心病的老年患者,均行标准12导联静息ECG检查,并将其慢性ST-T改变情况与冠脉造影结果进比较。结果以冠脉造影结果阳性判定为冠心病,分析ECG的ST-T改变情况,心电图阳性组90例,其中真阳性34例,占37.8%,假阳性56例,占62.2%;心电图阴性125例,其中假阴性72例,占57.6%,真阴性53例,占42.4%。结论静息心电图对冠心病的诊断价值并不高,临床依据持续性ST段压低诊断冠心病时应慎重。

Objective To explore the diagnostic value of routine resting ECG ST-T changes on coronary heart disease of old age.Methods 215 cases of elderly patients suspected coronary artery disease required to undergo coronary angiography were underwent the standard 12-lead resting ECG examination and ST-T changes of chronic conditions into the comparison with coronary angiography.Results Of 90 cases with ECG-positive,34 patients were true-positive.There were 37.8% of patients with coronary heart disease,56 cases were false-positive%.In 125 cases with ECG-negative,72 cases were a false negative.106 cases were true negative.Conclusion Because the diagnostic value of routine resting ECG on Coronary heart disease is not high,the clinical basis for diagnosis of persistent ST segment depression in coronary heart disease should be careful.
