四君子汤治疗AECOPD无创机械通气并发腹胀患者的临床观察 被引量:3

摘要 目的观察四君子汤治疗AECOPD无创机械通气并发腹胀患者的临床疗效。方法将20例患者随机分为对照组10例,中药治疗组10例,治疗2周,记录2组机械通气时间、腹胀持续时间、排便时间;治疗前后腹围、自主潮气量(sTv)、最大吸气负压(MIP)、气道平台压(PpLat)、气道峰压(PIP)。结果中药组机械通气时间、腹胀持续时间、排便时间;前后腹围、自主潮气量(sTv)、最大吸气负压(MIP)、气道平台压(PpLat)、气道峰压(PIP)与对照组比较有明显差异。结论四君子汤配合无创机械通气治疗COPD确实较常规西医治疗能更为有效的减轻腹胀程度;缩短机械通气及排便时间;降低气道峰压、平台压;提高自主潮气量及最大吸气负压,有效提高撤机成功率。 Objective To observe the therapeutic effect of Sijunzi decoction in the treatment of patients with AECOPD noninvasive mechanical ventilation concurrent abdominal distension.Methods Twenty patients were randomized into control group(n=10) and treatment group with traditional Chinese medicine(n=10),the treatment lasted for two weeks.Mechanical ventilation time,abdominal distension duration,defecate time of the two groups were recorded.Abdominal perimeter of before and after treatment,self-directed tide volume(sTv),maximal inspiratory negative pressure(MIP),platform pressure(Pplat),and peak inspiratory pressure(PIP) were also recorded.Results Compared with control group,the treatment group′s mechanical ventilation time,abdominal distension duration,defecate time,abdominal perimeter of before and after treatment,self-directed tide volume(sTv),maximal inspiratory negative pressure(MIP),platform pressure(Pplat),and peak inspiratory pressure(PIP) had obvious difference.Conclusion Compared with conventional western medicine treatment,Sijunzi decoction and noninvasive mechanical entilation with COPD can be more effective in relieving unobstruction degree,shortening mechanical ventilation and defecate time,lowering peak inspiratory pressure and platform pressure,and improving self-directed tide volume and maximal inspiratory negative pressure.
