细胞DNA定量分析在宫颈癌初筛中的应用研究及宫颈癌筛查的误区分析 被引量:1

摘要 目的通过将子宫颈癌及癌前病变筛查结果与活检结果比较,评价细胞DNA定量分析法在宫颈癌筛查中应用的可行性,分析目前防癌筛查误区。方法对4598人采用全自动DNA倍体定量分析检测进行早期宫颈癌筛查,检测结果为阳性者在阴道镜下取活体组织送病理确诊。结果 共检出91例阳性,包括1例癌,8例CINⅢ/CIS,32例CINⅡ以及50例CINⅠ。此外,在活检确诊的41例CINⅡ以上的高级别病变中,有2例显示Ⅲ度宫颈糜烂;4例为Ⅱ度宫颈糜烂;2例为Ⅰ度宫颈糜烂,另有1例纳囊,而宫颈光滑者有15例,息肉2例,其他1例。结论 DNA定量分析可作为宫颈癌筛查的手段,而是否罹患宫颈癌只通过妇科临床观察并不能确诊,需定期进行宫颈癌防癌筛查。 Objective To evaluate the application of DNA image cytometry on cervical cancer screening by comparing with biopsy result and analyze the misunderstanding in the screening program. Methods All 4598 cases were involved in this screening program and screened by DNA image cytometry. The positive cases accepted colposcopy for checking and being taken biopsy. Results There were 91 people who had the positive results, including 1 cancer, 8 CIN Ⅲ/CIS, 32 CIN 11 and 50 CIN Ⅰ. In those 41 cases with above CIN Ⅱ, 2 cases were cervical erosion Ⅲ, 4 cases were cervical erosion Ⅱ, 2 cases were cervical erosion Ⅰ. In addition, 1 case was Naboth's cyst, 2 cases were cervical polypus, but 15 cases with normal cervix. Conclusion DNA image cytometry can be a useful method to be applied in cervical cancer screening. To confirm a cervical cancer must be judged by routine screening but not by clinical symptoms.
