CT平扫对慢性阻塞性肺部疾病性肺动脉高压诊断初探 被引量:2

摘要 目的提高CT平扫对慢性阻塞性肺部疾病性肺动脉高压的诊断。方法测量176例肺动脉高压患者CT图像上的主肺动脉、右下肺动脉、降主动脉直径,胸椎椎体横径,并分析其相互关系。结果病例组:主肺动脉直径(32.8±4.0)mm;右下肺动脉直径(10.7±2.6)mm;主肺动脉直径/降主动脉直径比值1.4±0.4;主肺动脉直径/胸椎椎体横径比值1.3±0.3;对照组:主肺动脉直径(24.6±2.7)mm;右下肺动脉直径(7.7±1.7)mm;主肺动脉直径/降主动脉直径比值1.2±0.6;主肺动脉直径/胸椎椎体横径比值1.0±0.4。主肺动脉直径与右下肺动脉直径、主肺动脉直径/降动脉直径比值、主肺动脉直径/胸椎椎体横径比值呈正相关。结论 CT平扫对主肺动脉直径、主肺动脉直径/降主动脉直径比值、主肺动脉直径/胸椎直径比值作为评价肺动脉高压的指标。 Objective To improve the diagaosis of the pulmmary arthrial hypertension indued by chronic obstructive pulmonary disease.Methods On CT image measured on 176 cases of pulmonary hypertension patients of the main pulmonary artery,left pulmonary artery,the descending aorta diameter,transverse thoracic spinal body diameter,and their relationship.Results The case group: the main pulmonary artery was(32.8±4.0) mm in diameter,right lower pulmonary was(10.7±2.6) mm in diameter,the main pulmonary artery diameter/descending aorta diameter ratio 1.4±0.4.The main pulmonary artery diameter/thoracic vertebral horizontal diameter ratio was 1.3±0.3,the control group: the main pulmonary artery was(24.6±2.7) mm in diameter,right lower pulmonary was(7.7±1.7) mm in diameter,the main pulmonary artery diameter/descending aorta diameter ratio was 1.2±0.6,the main pulmonary artery diameter/thoracic vertebral horizontal diameter ratio was 1.0±0.4.With the main pulmonary artery diameter increase on the left,and pulmonary artery diameter,the main pulmonary artery/down artery diameter ratio in diameter,the main pulmonary artery diameter/thoracic vertebral horizontal diameter ratio of parturients increase.Conclusion CT flat on the main pulmonary artery saul diameter,the main pulmonary artery/descending aorta ratio,the main pulmonary artery/thoracic ratio measurement as evaluation index of pulmonary hypertension.
