2型糖尿病患者自我管理干预随机对照试验的系统综述

A Systematic Review of Self-management RCT Interventions among Patients with Type 2 Diabetes

目的本研究对国内2008—2010年公开发表的、较高质量的针对2型糖尿病患者自我管理干预的随机对照试验进行系统综述,为今后2型糖尿病患者自我管理研究和实践工作提供借鉴和参考。方法利用关键词检索技术和一定的纳入及排除标准,选取20项研究(25篇文献)在一般情况、干预措施和效果评价指标等方面进行系统综述。结果在20项研究中,包括单纯随机对照研究、整群随机对照研究、平行随机对照研究、整群平行随机对照研究,累计干预人群7 241例,对照人群6 696例,涉及9个国家或地区;主要在社区开展实施,由专业人员或社会网络人员提供自我管理支持,利用多种行为改变理论和信息技术;评价指标主要涉及健康状况、患者对干预的依从性、卫生资源利用情况和成本分析四个方面。结论建立一套系统、有效的以社区为基础的2型糖尿病患者自我管理模式显得至关重要。

Objective The study was a systematic review among patients with type 2 diabetes of self-management RCT interventions that were high-quality and had been published in 2008—2010,in order to provide evidence and experiences for further studies or routine works.Methods By using search methods and certain inclusive and exclusive criteria,20 studies（25 literatures） were involved into the study to review their general background,interventions,evaluation indicators,etc.Results These 20 studies included RCT,cluster RCT,parallel RCT and cluster parallel RCT,involving 7241 accumulated intervention persons and 6696 accumulated control persons from 9 countries or areas;most of the studies were implemented in communities and self-management supporters were professional persons or social-network persons using several behavior change theories and information technologies;the evaluation indicators were mainly on four aspects：health status,patients＇ compliance with interventions,utilization of health resources and cost.Conclusion It is of great importance to develop a systematic and effective community-based self-management model among patients with Type 2 diabetes.
